    J.01.E.B   Sulfonilamides of short action

    Pharmacodynamics:

    Argosulphan® is an antibacterial topical preparation that helps to heal wounds (burns, trophic, purulent, etc.), provides effective protection of wounds from infection, shortens the treatment time and preparation time of the wound for skin transplantation, in many cases leads to an improvement in the condition, eliminating the need for transplantation.

    Included in the cream sulfanilamide - sulfatiazole silver, is an antimicrobial bacteriostatic agent, has a broad spectrum of antibacterial bacteriostatic action against gram-positive and gram-negative bacteria.The mechanism of antimicrobial action sulfathiazole -ugnetenie growth and reproduction of bacteria associated with a competitive antagonism with para-aminobenzoic acid and digidropteroatsintetazy inhibition that leads to disruption of the synthesis of dihydrofolic acid and eventually its active metabolite - tetrahydrofolic acid required for the synthesis of purines and pyrimidines microbial cells. The silver ions present in the preparation enhance the antibacterial action of sulfanilamide - they inhibit the growth and division of bacteria by binding to the deoxyribonucleic acid of the microbial cell. In addition, silver ions weaken the sensitizing properties of sulfonamide.

    Due to the minimal resorption of the drug, it does not have a toxic effect.

    Pharmacokinetics:

    Contained in the preparation sulfatiazole silver, has a low solubility, as a result of which, after topical application, the concentration of the active substance in the wound is long maintained at the same level. Only a small amount of silver sulfatiazole appears in the bloodstream, after which it is acetylated in the liver.

    In the urine is in the form of inactive metabolites and partially unchanged. The absorption of silver sulfatiazole increases after application on extensive wound surfaces.

    Indications:

    - Burns of various degrees, of any nature (thermal, solar, chemical, electric shock, radiation, etc.); frostbite.

    - Bedsores; trophic ulcers of the lower leg of various genesis (including, with chronic venous insufficiency, obliterating endarteritis, blood supply disorders in diabetes mellitus, erysipelas, etc.);

    - Purulent wounds; minor domestic injuries (cuts, abrasions);

    - Infected dermatitis, impetigo, simple contact dermatitis, microbial eczema; streptostafilodermia.

    Contraindications:

    - Hypersensitivity to sulfatiazole and other sulfonamides.

    - Congenital insufficiency of glucose-6-phosphate dehydrogenase.

    - Prematurity, breast age up to 2 months (risk of developing "nuclear" jaundice).

    Pregnancy and lactation:The use of Argosulphan® during pregnancy and lactation is allowed in cases where the surface of the burn does not exceed 20% of the body surface and when, according to the attending physician, the potential benefit to the mother predominates over the possible risk forfetus and child.
    Dosing and Administration:

    Outwardly - as an open method, and under occlusive dressings.

    After cleansing and surgical treatment on the wound, the preparation is applied with the observance of sterility conditions and 2-3 mm 2-3 times a day 2-3 mm thick. The wound should be covered with cream during treatment. If part of the wound opens, you need to apply the cream additionally. The application of an occlusive dressing is possible, but is not mandatory. The cream is applied until the wound is fully healed or until the skin is transplanted. If the drug is applied to infected wounds, exudate may appear. Before applying the cream, it is necessary to wash the wound with a 0.1% aqueous solution of chlorhexidine or another antiseptic. The maximum daily dose is 25 g. The maximum duration of treatment is 60 days.

    Side effects:Allergic reactions, local reactions (burning, itching, skin hyperemia). With prolonged use, leukopenia, desquamatous dermatitis are possible.
    Overdose:Cases of overdose are not registered.
    Interaction:It is not recommended to be used together with other local medicines. Folic acid and its structural analogs are capable of weakening the antimicrobial action of sulfatiazole.
    Special instructions:

    With hepatic and / or renal insufficiency, it is necessary to control the concentration of sulfatiazole in the blood serum.

    Care should be taken when using in patients in a state of shock with extensive burns, because of the impossibility of collecting a full-fledged allergic anamnesis.

    Effect on the ability to drive transp. cf. and fur:The drug does not limit the psychophysical ability, the ability to drive vehicles and maintain machinery in motion.
