    D.10.A.F.51   Clindamycin in combination with other drugs

    Pharmacodynamics:

    The combination of benzoyl peroxide and the antibiotic clindamycin provides a dual action: anti-inflammatory and antibacterial.

    Benzoyl peroxide

    Reduces the amount of fatty acids and lipids on the skin surface. Has keratolytic and antibacterial action (in relation to Propionibacterium acnes). Has a moderate desquamative effect, reducing the appearance of acne and comedones.

    Clindamycin

    Antibiotic group of lincosamides, which have a wide spectrum of action. Inhibits protein synthesis in prokaryotic ribosomes. It is active against Gram-positive pathogenic and conditionally pathogenic microflora, including Staphylococcus epidermidis, Streptococcus spp., Mycoplasma spp., as well as anaerobic microflora, including clostridia.

    Pharmacokinetics:

    At local application the preparation is absorbed weakly and practically does not render resorptive action.

    Indications:

    It is used to treat draining acne, rosacea, with papulopustular eruptions. Positive effect occurs after one to two weeks, the course of treatment is 2-4 weeks.

    Contraindications:

    Burns, individual intolerance, children under 12 years.

    Carefully:

    Dermatitis, eczema, hypersensitivity.

    Pregnancy and lactation:

    Recommendations for FDA - Category C. It is used with caution during pregnancy and lactation in cases where the intended benefit exceeds the risk to the fetus and newborn.

    Dosing and Administration:

    It is applied externally before going to bed to clean, dry skin. The repeated course of treatment is carried out not earlier than in two weeks. From the thickness of the applied layer, the efficiency does not improve.

    Side effects:

    Local hyperemia, skin irritation and flaking, allergic reactions.

    Overdose:

    Cases of overdose are not described.

    Treatment is symptomatic.

    Interaction:

    Isotretinoin and tretinoin reduce the effectiveness of the drug and increase skin irritation. If it is necessary to combine treatment, it is recommended to use medicines at different times with an interval of at least 12 hours.

    Erythromycin shows antagonism to clindamycin.

    Special instructions:

    In the treatment it is recommended to avoid applying to the mucous membranes (lips and conjunctiva).Benzoyl peroxide stimulates the mitosis of epidermal cells, which leads to hyperkeratosis and acanthosis; redness and flaky skin causes a period of apparent worsening during the first weeks of treatment.

    When skin irritation is recommended, temporary discontinuation or complete cancellation of the drug.
