    D.05.A.X   Other preparations for the treatment of psoriasis for external use

    Pharmacodynamics:

    Synthetic analogue of the most active metabolite of natural vitamin D3. Causes a dose-dependent inhibition of the proliferation of keratinocytes (significantly elevated in patients with psoriasis) and accelerates their morphological differentiation. Has an insignificant effect on the metabolism of calcium in the body.

    Pharmacokinetics:

    Transdermal absorption of calcipotriol is from 1% to 5% of the dose. Calcipotriol undergoes rapid biotransformation in the liver with the formation of pharmacologically inactive metabolites. Half-life very short.

    Indications:Psoriasis is vulgar, including chronic psoriasis of the scalp.

    Contraindications:

    Hypersensitivity, psoriasis in the progressing stage, pustular psoriasis, diseases accompanied by a violation of calcium metabolism. Children under 12 years (cream, solution).Do not use on the face for application.

    Carefully:

    Hypercalcemia, hypercalciuria (including predisposition), hypervitaminosis D, nephrolithiasis (including in the anamnesis), the age to 18 years and over 65 years old.

    Pregnancy and lactation:

    Pregnancy and lactation should be avoided.

    Dosing and Administration:

    Outwardly.

    Adults (ointment, cream, solution) - apply to affected areas of the skin 2 times a day. The maximum daily dose of 15 g, total for a week is not more than 100 g (for cream and ointment), 60 ml (solution). The course of treatment should not exceed 6-8 weeks.

    If the solution is simultaneously applied with cream or ointment, the total dose should not exceed 5 mg per week (60 ml of solution and 30 g of cream or ointment or 30 ml of solution and 60 g of cream or ointment).

    Children (ointment only!) - Apply to affected areas of the skin 2 times a day. For children over 12 years, the maximum weekly dose should not exceed 75 g, for children from 6 to 12 years - 50 g. The course of treatment should not exceed 6-8 weeks.

    Side effects:

    From the skin and subcutaneous fat: often - irritation, rash (scaly, erythematous, maculopapular, pustular, bullous), burning sensation, dryness and scaling of the skin, itching, erythema,contact dermatitis; rarely - exacerbation of psoriasis, eczema; very rarely - reversible hyperpigmentation, reversible depigmentation, reversible development of photosensitivity, periorbital or edema of the face.

    From the side of metabolism: very rarely - hypercalcemia, hypercalciuria.

    From the immune system: very rarely - allergic reactions, hives, angioedema.

    Overdose:

    Symptoms: Hypercalcemia (abdominal pain, constipation, depression, unusual fatigue, increased blood pressure, loss of appetite, weight loss, thirst, nausea, vomiting, hypercalciuria).

    Treatment: cancellation of the drug, intensive hydration (oral and parenteral) of the body and forced diuresis, potassium chloride (to prevent hypokalemia), glucocorticoids, hemodialysis (with renal failure); monitoring the level of electrolytes, daily diuresis.

    Interaction:

    Do not use together with topical preparations containing salicylic acid.

    Special instructions:

    Do not apply to large surfaces of lesions (more than 30% of the surface of the skin).

    When using the drug, avoid wearing compressive clothing.After applying to the area of ​​lesions, you should carefully wash your hands to avoid getting the drug on your face, mucous membranes or healthy skin areas. It is recommended to limit or avoid excessive exposure to sunlight.

    When combining calcipotriol with PUVA therapy, apply the drug to the skin at least 2 hours before irradiation.

    With psoriasis of the scalp, before using the drug, comb hair thoroughly, remove flakes, do not apply on healthy areas of the skin and elements of the acute stage of psoriasis.
