    Pharmacodynamics:

    Inhibits the synthesis of β- (1,3) -D-glucan (β-(1,3) -D-glucan is not included in mammalian cell membranes), one of the main components of the yeast cell wall and mycelial fungi.

    Effective against pathogenic fungi of the genus Aspergillus (including Aspergillus flavus, Aspergillus niger, Aspergillus fumigatus, Aspergillus terreus and Aspergillus nidulans) and Candida spp. (including Candida albicans, Candida dubliniensis, Candida glabrata, Candida guilliermondii, Candida kefyr, Candida krusei, Candida lipolytica, Candida lusitaniae, Candida parapsilosis, Candida rugosa and Candida tropicalis), resistant to amphotericin B, fluconazole or flucytosine, which have a different mechanism of action.

    Pharmacokinetics:

    After intravenous infusion, the maximum concentration in the blood plasma is reached after 1 hour. The connection with plasma proteins is 97%.

    Penetrates through the placental barrier. Metabolism in the liver.

    The half-life is 9-11 hours. Elimination by the kidneys.

    Indications:

    It is used to treat "ex juvantibus" patients with febrile neutropenia in case of suspected invasive candidiasis. Used as a remedy for oropharyngeal and esophageal candidiasis.

    Contraindications:

    Individual intolerance, children under 18 years.

    Carefully:

    Moderate hepatic insufficiency, hypersensitivity.

    Pregnancy and lactation:

    Recommendations for FDA - Category C. Contraindicated in pregnancy and lactation.

    Dosing and Administration:

    Intravenously slowly, for one hour or more, in a dose of 50-70 mg once a day.

    The highest daily dose: 70 mg.

    The highest single dose: 70 mg.

    Side effects:

    Central and peripheral nervous system: headache, hyperthermia.

    Respiratory system: shortness of breath, bronchospasm, rarely - pulmonary edema.

    Hemopoietic system: anemia, thrombocythemia, eosinophilia, neutropenia, leukopenia.

    The cardiovascular system: tachycardia, reddening of the face, peripheral edema, phlebitis, thrombophlebitis, an increase in the prothrombin index.

    Digestive system: nausea, vomiting, diarrhea, disorders function of the liver.

    Dermatological reactions: hyperhidrosis, itching, rash.

    Urinary system: leukocyturia, polyuria, proteinuria, microhematuria.

    Allergic reactions.

    Overdose:

    Increased side effects.

    Treatment is symptomatic.

    Interaction:

    Simultaneous use with cyclosporin is possible transient increase in ALT and AST.

    Dexamethasone, nevirapine, phenytoin, carbamazepine accelerate the elimination of caspofungin.

    Special instructions:

    Empirical therapy is performed until the neutropenia is completely resolved.
