    A.01.A.A.01   Sodium Fluoride

Pharmacodynamics:Fluorine ions affect the mineralization of hard tooth tissues during development; form the most stable forms of apatite - fluorapatite. The drug stimulates inhibition of bone resorption, anti-rheumatic action: stimulation of osteoblasts, replacement of hydroxyapatite with fluorapatite in bone trabeculae. It has an anticarious effect due to a decrease in the cariogenic activity of plaque (especially in children and adolescents), shows bactericidal action.
Pharmacokinetics:Absorption is fast and complete. Biotransformation in the liver. The maximum concentration is 30-60 minutes. Elimination by the kidneys (50%), in small amounts, the drug is excreted with feces and sweat. At any dose of 50% of the fluoride that is received accumulates in the hard tissues of the tooth and bone tissue. Cumulation in the bones and teeth in the growth stage.
Indications:Primary osteoporosis: postmenopausal, presenilny, senile, idiopathic.

Steroid osteoporosis - prevention and treatment.

    Local osteopathy.

Prevention of caries in children and adults, with fluoride content in drinking water below 0.6 mg / ml.

    XI.K00-K14.K02   Dental caries

    XIII.M80-M85.M81.9   Osteoporosis, unspecified

    XIII.M80-M85.M81.4   Medicinal osteoporosis

    XIII.M86-M90.M89.5   Osteolysis

    Contraindications:Hypothyroidism.

    Individual intolerance.

Stomach ulcer and duodenal ulcer in the stage of exacerbation.

Hepatic and / or renal insufficiency.

    Carefully:No data.
Pregnancy and lactation:Recommendations FDA category C. Contraindicated in use during pregnancy and breastfeeding.
Dosing and Administration:For osteoporosis, inside is 0.015-0.02 g 3-4 times a day daily. Treatment is long, for 1-4 years continuously or in courses - 3 months of treatment, 2-3 months break.

For the prevention of steroid osteoporosis, one tablet per day is prescribed.

For prophylaxis of caries during the entire period of formation of teeth: for children from 2 to 6 years old long tablets of 0,0011 grams, over 6 years - 0,0022 grams per day; over 16 years old - mouthwash 0.05-0.2% solution after eating and brushing teeth.

Side effects:Digestive system: nausea, vomiting, anorexia, diarrhea.

Nervous system: increased fatigue, headache, visual impairment.

Pain in joints for 3-6 months after treatment. More often in the ankle, knee and hip joints (up to a temporary inability to move).

Osteosclerosis, ossification of the attachment sites of tendons and ligaments.

    Hypothyroidism (with prolonged use).

Allergic reactions, rhinitis, eosinophilia, skin rash.

Fluorosis (violation of the formation and calcification of enamel, the formation of yellow and brown spots, increased fragility and abrasion of the teeth).

Overdose:The lethal dose is unknown, presumably 5-10 g sodium fluoride for adults and 5 mg / kg for children. Overdose develops bloody vomiting, nausea, dizziness, hypersalivation, anorexia, diarrhea, drowsiness, lacrimation, abdominal pain, pain in the lower extremities, arthralgia, fatigue and weakness, miosis, visual impairment, exophthalmos, tremor, convulsions, fever, tachycardia, lowering blood pressure, respiratory failure, stopping breathing.

Treatment: the administration of large quantities of liquid, calcium gluconate (20 ml 10-20% solution) or calcium lactate, milk (for the precipitation of fluorides).

Interaction:Antacids, preparations containing Ca2 +, Mg2 +, Al3 +, slow down absorption and increase fluoride excretion, reduce its effect; they should be taken no earlier than 2 hours before taking sodium fluoride.

Vitamins A and D - development of ectopic calcification.

Special instructions:Patients with blood diseases during the period of therapy require regular monitoring of blood tests. During the systemic application it is necessary to conduct an X-ray examination annually to evaluate the effectiveness of therapy and correction of treatment, as well as for the early detection of fluorosis. When choosing a dose of the drug should take into account the fluoride content in drinking water.
