    Pharmacodynamics:The drug is responsible for the binding of iron ions and suppression of the activity of DNA-gyrase and topoisomerase IV; violation of DNA synthesis of microorganisms. Has antibacterial and bactericidal effects. Affects most Gram-negative microorganisms: Pseudomonas aeruginosa, Escherichia coli, strains Proteus spp., Klebsiella spp., Shigella spp., Salmonella spp.; Some Gram-positive microorganisms, in particular Staphylococcus aureus.
    Pharmacokinetics:Connection with plasma proteins - 15%. Half-life is 2-3 hours. The maximum concentration is achieved after 2 hours. Elimination by the kidneys (50-70%).
    Indications:Acute and chronic infectious diseases of the urinary tract caused by sensitive microorganisms: pyelonephritis, cystitis, urethritis; prostatitis; prevention of infections during catheterization, endoscopy and other instrumental interventions.

    Contraindications:Hypersensitivity.

    Chronic renal failure (creatinine clearance less than 10 ml / min).

    Liver failure.

    Pregnancy (I and III trimester).

    Pregnancy and lactation:Contraindicated in I and III trimesters of pregnancy. The FDA action category is not defined. For the duration of treatment, breastfeeding should be discontinued.
    Dosing and Administration:Pyelonephritis, urethritis, cystitis, prostatitis: inside adults, 400 mg 2 times a day (morning and evening) for 10 days.

    With staphylococcal infections: 400 mg 3 times a day (single dose can be increased to 600 mg) for 10 days or more.

    Urinary tract infections in women older than 58 years: 400 mg twice a day for 5 days.

    Intravaginal. In recurrent acute urinary tract infections, 1 oral vaginal suppository is prescribed for women simultaneously with oral administration for 7-10 days.

    Side effects:Digestive system: nausea, vomiting, diarrhea, abdominal pain.

    Allergic reactions: skin rash, photosensitivity.

    Overdose:Not described. Treatment is symptomatic.
    Interaction:Means, alkalinizing urine, increase the activity of pipemidic acid.

    With simultaneous application, the clearance of theophylline and caffeine is significantly reduced, and the half-life of them is increased.

    Special instructions:In patients with deficiency of glucose-6-phosphate dehydrogenase can cause hemolytic anemia. Very rarely there can be large seizures.

    Caution should be exercised in prescribing elderly people (over 70 years) and patients with renal dysfunction, patients with a history of cerebral hemorrhage, cerebral circulation disorder and cerebral vascular spasms, as well as convulsions.

    In the treatment avoid exposure to sunlight and quartz due to possible photosensitization.
