A.11.H.A.06   Pyridoxalphosphate

Pharmacodynamics:

A drug that affects metabolism. It is a coenzyme form of vitamin B6 (pyridoxine) and has the properties of vitamin B6. Carries out decarboxylation and reamination of amino acids. Reduces the concentration of cholesterol and lipids, increases the amount of glycogen in the liver, improves the detoxification properties of the liver, participates in the exchange of histamine. Stimulates metabolic processes in the skin and mucous membranes.

The advantage of pyridoxal phosphate over other vitamin B preparations6consists in the fact that its therapeutic effect develops rapidly and it can be used in conditions resistant to the action of pyridoxine, when the phosphorylation of the latter is disturbed.

Pharmacokinetics:

Pyridoxal phosphate is rapidly absorbed from the digestive tract. It penetrates through the placental barrier and is excreted in breast milk.

It is excreted in the urine.

Indications:

Prevention and treatment of hypovitaminosis B6, including in children older than 5 years, including the following diseases and conditions: hypochromic and microcytic anemia, leukopenia, parkinsonism, chorea, hepatitis, dermatitis (including atopic), diffuse neurodermatitis, exudative diathesis, psoriasis.

In the complex therapy of cerebrovascular insufficiency, lesions of the peripheral nervous system (radiculitis, neuritis, neuralgia), chronic heart failure,chronic hepatitis, cirrhosis of the liver, toxicosis of the first half of pregnancy (mild and moderate).

For the prevention of liver damage, the development of neuritis and other complications against the background of the use of anti-tuberculosis drugs.

Contraindications:

Hypersensitivity, hepatic insufficiency.

Carefully:

With caution should be used in patients with peptic ulcer disease of the stomach and duodenum (due to the possible lowering of the pH of gastric juice), as well as with IHD.

Pregnancy and lactation:

Possible use of the drug during pregnancy according to the indications in the recommended doses.

Data on the possibility and safety of the drug during lactation there.

Dosing and Administration:

Inside 10-15 minutes after eating.Single dose for adults 0,02-0,04 g, daily 0,04-0,16 g. A single dose for children 0,01-0,02 g, daily 0,02-0,06 g. The course of treatment from 10 up to 30 days or more (depending on the nature of the disease and the effectiveness of therapy).

Subcutaneously, intramuscularly, intravenously. The solution is prepared immediately before use (the contents of the ampoule are diluted with 1-2 ml of water for injection). The dose, frequency of application and duration of the course are determined individually. Adults to 0,005-0,01 g 1-3 times a day, children, depending on the age to 0,005-0,01 g per day.

Side effects:

Pyridoxal phosphate is usually well tolerated. Allergic reactions (skin rashes), acidity of gastric juice are possible.

Overdose:

There are currently no data on drug overdose cases.

Interaction:

Cycloserine, penicillamine with simultaneous use reduce the effect of pyridoxal phosphate.

Pyridoxal phosphate with simultaneous application weakens the antiparkinsonian activity of levodopa.

Pyridoxal phosphate is pharmaceutically incompatible with B vitamins1 and B12.
