H.03.B.B.02 Thiamazole
Pharmacodynamics:It blocks peroxidase, inhibits iodination of tyrosine, reducing the synthesis of tetraiodothyroxine and tetraiodothyronine - thyroxine. Reduces the basal metabolism, accelerates the excretion of the thyroid gland of iodides, and therefore increases the reciprocal activation of the synthesis of the pituitary thyroid-stimulating hormone, leading to hyperplasia of the thyroid gland.
Pharmacokinetics:After ingestion, up to 98% is absorbed in the gastrointestinal tract. The maximum concentration in blood plasma is reached after 1 hour. Virtually does not bind to plasma proteins, it concentrates in the thyroid gland, where it undergoes primary metabolism. It penetrates the placental barrier and enters the breast milk.
Metabolism in the liver and kidneys.
The elimination half-life is 4 hours. Elimination by the kidneys (70%) and with feces (30%).
Pharmacological parameters do not depend on the functions of the thyroid gland.
Indications:It is used for the treatment of diffuse toxic goiter, relief of thyrotoxic crisis, used in combination therapy with radioactive iodine and thyroid hormone preparations.

Contraindications:
Leukopenia, granulocytopenia, individual intolerance.
Carefully:Patients with large thyroid glands, leading to a narrowing of the trachea.

Dosing and Administration:
Use in children
In the morning, after eating 300-500 mg.
Adults
Inside, 20-40 mg / day for 2-4 admission.
The highest daily dose: 40 g.
The highest single dose: 10 g.
Side effects:Central and peripheral nervous system: headache, rarely - polyneuropathy.
The system of hematopoiesis: aplastic anemia, agranulocytosis, leukopenia, rarely generalized lymphadenopathy
Digestive system: nausea, vomiting, hepatitis, cholestatic stomach.
Musculoskeletal system: arthralgia, myalgia.
Dermatological reactions: alopecia, hyperpigmentation of the skin.
Urinary system: rarely - jade.
Allergic reactions.
Overdose:Hypothyroidism.
Treatment is symptomatic.
Interaction:Lack of iodine increases, and excess - weakens the effect of thiamazole.
β-adrenoblockers, lithium preparations, amiodarone, reserpine, - enhance the effects of thiamazole.
Simultaneous use with sulfonamides, myelotoxic agents, metamizole sodium increases the risk of developing leukopenia.
The antithyroid effect increases with simultaneous application with gentamycin.
Special instructions:Monitoring the blood formula.
For 1-2 weeks before the operation, it is recommended to replace thiamazole with iodine-containing preparations.
