    Pharmacodynamics:

    Synthetic imidazole for topical application.

    Inhibition of sterol-14-demethylase (cytochrome P450 isoenzyme) of fungal cells: a disruption in the synthesis of ergosterol involved in the construction of fungal membranes; disruption of the accumulation of 14α-methylsterols in the dense packing of hydrocarbon chains of phospholipids necessary for the normal functioning of membrane proteins, including ATPase and respiratory chain enzymes. AT Candida albicans - inhibition of the transformation of blastophores into an invasive mycelium.

    Pharmacokinetics:

    Absorption of 5.5% with intravaginal application; biotransformation in the liver; half-life - 21-24 hours; elimination of kidneys and feces.

    Indications:

    Candidiasis of the vagina caused by Candida albicans.

    Contraindications:Hypersensitivity, menstrual period, pregnancy (I trimester).
    Carefully:Lactation.
    Pregnancy and lactation:Recommendations Food and Drug Administration (US Food and Drug Administration) category C (intravaginal). Adequate and well-controlled studies in the first trimester were not carried out on a human; in the II and III trimesters, violations from the fetus and the mother were not detected.In animals, induced embryotoxicity, but not teratogenicity. Do not apply in the first trimester!
    There is no information on the penetration into breast milk. Violations are not registered.
    Dosing and Administration:Intravaginal. The content of one applicator (about 5 grams) is inserted into the vagina at any time of the day only once.
    Side effects:Locally: itching, burning sensation in the vagina, skin rash in the area of ​​the external genitalia; allergic reactions; soreness and swelling of the vaginal mucosa, pain / spasms in the lower abdomen.
    Overdose:Cardiopulmonary insufficiency.

    Treatment is symptomatic.

    Interaction:Alfentanil, eletriptan - decrease in the clearance of these funds.

    Astemizole, terfenadine, cisapride, pimozide - increasing their concentration in the blood, lengthening the interval Q-T (sudden arrhythmic death possible). Joint use is contraindicated!

    Atorvastatin, lovastatin, simvastatin - increase in their concentrations in the plasma, the development of rhabdomyolysis. Joint use is contraindicated!

    Budesonide, dexamethasone, methylprednisolone - slowing the metabolism of glucocorticosteroids.

    Buspirone - a significant increase in the concentration of buspirone in the blood.

    Busulfan, docetaxel, vinca alkaloids - inhibition of the metabolism of these agents.

    Warfarin - strengthening of anticoagulant action.

    Hypoglycemic agents (sulfonylurea preparations, for example glibenclamide) - increasing their concentration in the blood, the development of hypoglycemia.

    Digoxin - increased concentration in the blood and toxicity.

    Dysopyramide - risk of lengthening the interval Q-T.

    Indinavir, ritonavir, saquinavin - a mutual increase in plasma concentrations.

    Macrolides (erythromycin, clarithromycin) - increased concentration and toxicity of butoconazole.

    Polyene antifungal agents, including amphotericin B, reduce the activity of polyene antibiotics.

    Rifampicin, isoniazid - a decrease in the concentration of azoles in the blood, inefficiency or exacerbation of the infection.

    Phenytoin - increasing its concentration in the blood, efficiency and toxicity.

    Quinidine - increasing its concentration in the blood and the risk of cardiovascular complications. Joint use is contraindicated!

    Cyclosporine, tacrolimus - increase of their toxicity.

    Special instructions:

    If clinical symptoms persist after the treatment is complete, a microbiological study should be repeated to confirm the diagnosis.

    If there is irritation of the vaginal mucosa or painful sensations, the use of butoconazole should be discontinued.
