АТХ:

B.05.A.A   Blood plasma preparations and plasma-substituting drugs

Pharmacodynamics:

Solutions for parenteral administration. Pharmacological action - plazmozameschayuschee, detoxification, antiaggregational.

Binds and removes toxins. Reduces the viscosity of the blood.

Pharmacokinetics:

Dextrose: completely metabolized, not eliminated (appearance in the urine - pathology). Biotransformation in the liver through pyruvate or lactic acid to water and carbon dioxide with the release of energy.

Povidone: elimination by the kidneys.

Indications:

Shock (post-traumatic, postoperative, burn, hemorrhagic).

Intoxication in the following conditions: toxic diseases of the digestive tract (shigellosis, dyspepsia, salmonellosis), burn, radiation and hemolytic disease, peritonitis, intestinal obstruction, thyrotoxicosis, liver disease, sepsis, pneumonia; acute myocardial infarction, toxemia of newborns, gestosis.

XVIII.R50-R69.R57.8   Other types of shock

XVIII.R50-R69.R57.1   Hypovolemic shock

Contraindications:

Hypersensitivity, chronic heart failure IIb-III stage, bronchial asthma, respiratory failure.

Hemorrhagic stroke, phlebotrombosis, thromboembolism, diabetes mellitus (the drug contains dextrose); condition after a head injury;oliguria, anuria.

Carefully:

With caution: chronic renal failure (oliguria, anuria).

Pregnancy and lactation:

Category of recommendations for FDA is not defined. Adequate and well-controlled studies in humans and animals have not been conducted.

There is no information on the penetration into breast milk.

Dosing and Administration:

The dosage regimen is individual, depending on the age and condition of the patient.

Intravenously drip through a system with a filter at a rate of 20-40 drops / min. Before administration, the solution is warmed to body temperature. The dose depends on the age of the patient and the severity of intoxication. Children - 2.5 ml / kg of body weight; the maximum single dose for infants is 50 ml, for children 2-5 years - 70 ml, for children 6-9 years - 100-150 ml, for children 10-15 years - 200 ml. For adults, the maximum dose is 400 ml. The drug is administered 1-2 times a day for 1-10 days.

Side effects:

Allergic reactions of varying severity (up to the development of anaphylactic shock).

Overdose:

Dextrose

Symptoms: hyperglycemia, glucosuria, hyperglycemic hyperosmolar coma, hyperhydration, disturbance of water-electrolyte balance.

Treatment: stopping the introduction of dextrose, the appointment of insulin, symptomatic therapy.

Povidone. Allergic reactions, nausea, vomiting.

Interaction:

Povidone - interactions are possible due to adsorbing properties.

Dextrose - possible pharmaceutical and / or pharmacodynamic incompatibility in combination with other drugs, visual compatibility control is necessary.

Special instructions:

Monitoring of blood pressure, kidney function.

Before the start of the infusion, the tightness and integrity of the package should be checked.

Povidone as a filler is contained in some preparations for parenteral administration; cases of povidone deposition in various tissues with subsequent damage and morbidity, liver damage were noted.
