  • Tocopherol (vitamin E)
  • Pharmacodynamics:

    Fat-soluble vitamin, the function of which remains unclear until the end. As an antioxidant, inhibits the development of free radical reactions, prevents the formation of peroxides that damage cellular and subcellular membranes, which is important for the development of the body. Together with selenium, it inhibits the oxidation of unsaturated fatty acids (a component of the microsomal electron transfer system), prevents the hemolysis of erythrocytes. It is a cofactor of some enzyme systems.

    Pharmacokinetics:

    Absorption from the duodenum (necessary presence of bile salts, fats, normal functioning of the pancreas) - 20-40%. When the dose is raised, the degree of absorption decreases. Time to reach the maximum concentration is 4 hours. It is deposited in all organs and tissues, especially in adipose tissue, penetrates through the placenta in insufficient quantities: 20-30% of the concentration in the blood of the mother penetrates into the fetal blood. Penetrates into breast milk. Excretion mainly through the gastrointestinal tract, with bile - more than 90%, less than 6% is excreted by the kidneys in the form of glucuronides and other metabolites.

    Indications:

    Treatment of hypo- and avitaminosis E.

    Contraindications:

    - Hypersensitivity;

    - acute myocardial infarction;

    - childhood.

    Carefully:

    With caution should be used in severe cardiosclerosis, after a heart attack, with an increased risk of thromboembolism, as well as hypoprothrombinemia (against a background of vitamin K deficiency - may increase with a dose of vitamin E more than 400 mg).

    Pregnancy and lactation:

    Vitamin E is used during pregnancy and during breastfeeding only as prescribed by the doctor.

    Dosing and Administration:

    Capsules 200 mg: adults 1-2 capsules per day.

    When taking large doses - diarrhea, epigastric pain, creatinuria, dyspepsia.

    If there are side effects, the dose of the drug should be reduced or canceled.

    Overdose:

    Symptoms: when taken for a long period in doses of 400-800 mg / day - blurred vision, dizziness, headache, nausea, unusual fatigue, diarrhea, gastralgia, asthenia; when taking more than 800 mg / day for a long period - an increased risk of bleeding in patients with hypovitaminosis K, a disorder of thyroid hormone metabolism, a sexual function disorder, thrombophlebitis, thromboembolism, necrotic colitis, sepsis, hepatomegaly, hyperbilirubinemia, renal failure, haemorrhage a covering of an eye, a hemorrhagic insult, an ascites, a hemolysis.

    Treatment: drug cancellation; prescribe glucocorticoids, which accelerate the metabolism of vitamin E in the liver; appoint Vikasol to reduce the risk of hemorrhages.
    Interaction:

    Enhances the effect of steroid and non-steroidal anti-inflammatory drugs, antioxidants.

    Increases the efficiency and reduces the toxicity of cardiac glycosides.

    The appointment of vitamin E in high doses can cause vitamin A deficiency in the body.

    Simultaneous use of vitamin E at a dose of more than 400 mg / day with anticoagulants (coumarin and indanedione derivatives) increases the risk of hypoprothrombinemia and bleeding.

    Colestramine, colestipol, mineral oils reduce the absorption of alpha-tocopherol acetate.

    With the simultaneous use of vitamin E with cyclosporin, the absorption of the latter increases.

    Special instructions:

    It is necessary to strictly follow the dosing regimen.

    With congenital bullous epidermolysis in areas affected by allopecia, white hair may begin to grow.

    With long-term use of the drug and / or the need to prescribe repeated courses of therapy, it is recommended to periodically monitor blood coagulation indicators, as well as the level of cholesterol in the blood.
