Pharmacodynamics:

It is a combined antibacterial preparation of topical application. Contains two antibiotics, which have bactericidal action: neomycin and bacitracin. Bacitracin is a polypeptide antibiotic, inhibits the synthesis of the bacterial cell membrane, is particularly active against Gram-positive (Streptococcus spp., incl. beta-hemolytic streptococcus, Staphylococcus spp.) and some gram-negative microorganisms. Neomycin violates the process of protein synthesis in bacterial cells.

Neomycin is an antibiotic-aminoglycoside, which inhibits the synthesis of bacterial proteins. It is active against gram-positive and gram-negative bacteria.

By using a combination of these two antibiotics, a broad spectrum of action of the drug and synergism of action against a number of microorganisms, for example staphylococci, is achieved.

Pharmacokinetics:

It is not absorbed into the blood, but accumulates in the skin at the site of application in sufficiently large concentrations. Systemic absorption is possible only when applied to large areas of damaged skin.

Indications:

Otology: notitis externa.

Dermatology: infection of the wound surface, bruises, prevention of purulent infection, epidermal pyoderma, gangrenous ecthima.

Ophthalmology: blepharitis, acute conjunctivitis, keratoconjunctivitis, ulceration of the cornea, blepharoconjunctivitis, dacryocystitis, meibomite.

Contraindications:

Perforation of the tympanic membrane.

Extensive skin lesions (risk of ototoxic effect, accompanied by hearing loss).

Severe impairment of the excretory function of the kidneys (due to cardiac or renal insufficiency).

The existing diseases of cochlear-vestibular apparatus (if systemic absorption of the drug is possible).

Hypersensitivity to bacitracin, neomycin or other aminoglycosides.

Do not use powder to treat the eyes.

Carefully:

Presence of chronic kidney diseases.

Pregnancy and lactation:

Not classified Food and Drug Administration (US Food and Drug Administration). Application during pregnancy and lactation not recommended. The use of the drug during pregnancy and during lactation is possible only if the intended benefit to the mother exceeds the potential risk to the fetus or baby (after consulting a doctor).

Dosing and Administration:

Powder in aerosol: application to wound surfaces 1-3 times a day.

Ointment: application to the affected surface 2-4 times a day.

Eye Ointment: Apply to the eyelid / conjunctiva with a thin layer 2-5 times a day.

Solution: 1-2 drops in the lower eyelid or auditory meatus 4-5 times a day or 2 drops every hour, if a serious infection. When improving, reduce the number of applications per day.

Side effects:

Allergic reactions: with prolonged use - redness, dry skin, skin rash, itching. In general, allergic reactions proceed according to the type of contact eczema.

System Effects: when extensive skin lesions should take into account the possibility of absorption of the drug and the development of oto- and nephrotoxic effects and disorders of neuromuscular conduction, superinfection.

When topically applied to the skin, mucous membranes and wound surfaces, the drug is usually well tolerated.

Overdose:

There is no information.

Interaction:

If there is a systemic absorption of the drug, it can aggravate the nephrotoxicity of other drugs (cephalosporins, aminoglycosides). When used simultaneously with ethacrynic acid or furosemide, the risk of oto and nephrotoxic reactions increases.

With simultaneous use with opioid analgesics, anesthetics and muscle relaxants, the risk of neuromuscular blockade increases.

There were no incidents of incompatibility between bacitracin and neomycin.

Special instructions:

The solution for external use must be prepared immediately before application in aseptic conditions. Ready the solution is stored for 3 weeks.

With the development of allergic reactions and superinfection, the drug should be canceled.

Avoid contact with the eyes.

When used in doses that are significantly higher than recommended, due to possible absorption, attention should be paid to symptoms indicating nephro- or ototoxic reactions.

Since the risk of toxic effects increases with decreasing liver and / or kidney function, blood and urine tests should be performed in patients with hepatic and / or renal insufficiency together with audiometric testing before and during drug therapy.
