Pharmacodynamics:

Antiepilepticheskoe means, practically does not have a hypnotic effect. Enhances inhibitory GABAergic effects in the central nervous system, especially in the thalamus, the ascending activating reticular formation of the brainstem at the level of intercalary neurons. Reducing the permeability of nerve fiber membranes for Na+, reduces the spread of impulses from the focus of epileptic activity. The effect occurs after 20-60 minutes after oral administration.

Pharmacokinetics:

Rapidly metabolized in the body, releasing phenobarbital, which has antiepileptic effect.The connection with plasma proteins is weak. Creates high concentrations in the brain, liver and kidneys. Penetrates through the histohematological barriers and into breast milk. The half-life is 3-4 days. It is excreted by the kidneys, both in an unchanged form, and in the form of metabolites.

Indications:

Epilepsy of various genesis, generalized and partial seizures, epileptic syndrome. Focal seizures, including Jackson epilepsy, kozhevnikovskaya epilepsy. Small seizures (without convulsions).

Contraindications:

Hypersensitivity, hepatic insufficiency, renal failure, heart failure II-III degree, porphyria, anemia, bronchial asthma, respiratory failure, diabetes mellitus, hyperthyroidism, adrenal insufficiency, hyperkinesia, depressive states (with suicidal attempts), children under 3 years , pregnancy (I and III trimester), lactation period.

Dosing and Administration:

Children: For children 3-6 years old single dose - 0.025-0.05 g, daily - 0.1-0.15 g; 7-10 years old single - 0,05-0,1 g, daily - 0,15-0,3 g; 11-14 years old single - 0.1 g, daily - 0.3-0.4 g; maximum single dose for older children - 0.15 g; the maximum daily - 0.45 g.

Side effects:

Addiction, drug dependency, the syndrome of "cancellation", bronchospasm, reduction in blood pressure, thrombocytopenia, anemia, thrombophlebitis, drowsiness, lethargy, confusion, headache, loss of appetite, difficulty in speech, ataxia, nystagmus, slow mental reactions; allergic reactions.

Overdose:

Symptoms: depression of the central nervous system functions (drowsiness, impairment of vision, ataxia, dysarthria, nystagmus) up to coma, depression of the respiratory center, lowering blood pressure, renal dysfunction, headache, severe weakness, an increase or decrease in body temperature, agitation, pupillary constriction, tachy or bradycardia, cyanosis, hemorrhages in the pressure field, lung edema. In chronic intoxication - irritability, weakening the capacity for critical evaluation, sleep disturbances, confusion.

Treatment: gastric lavage, activated charcoal, symptomatic

therapy.

Interaction:

Enhances the effect of narcotic analgesics, means for general anesthesia, neuroleptics, tranquilizers, tricyclic antidepressants, ethanol, hypnotics, reduces - paracetamol, anticoagulants, tetracyclines, griseofulvin, glucocorticosteroids, mineralocorticoids, cardiac glycosides, quinidine, vitamin D, xanthines.

Special instructions:

When switching to treatment with benzonal in patients who have previously taken other barbiturates, sleep disturbance is possible, which is eliminated by the appointment of phenobarbital (0.05-0.1) g or other hypnotics for the night. Currently, the treatment of patients with epilepsy begins with other antiepileptic drugs (valproic acid, carbamazepine and others) with less pronounced side effects. Benzobarbital prescribe, if these funds are not effective, more often as part of combination therapy.

During treatment, do not consume alcoholic beverages.
