    Pharmacodynamics:

    Benzobarbital is an antiepileptic agent. The drug almost completely lacks a hypnotic effect.

    Benzobarbital increases the inhibitory GABA-ergic effect on the central nervous system, especially in the thalamus, the ascending activating reticular formation of the brainstem at the level of intercalary neurons.

    The drug reduces the permeability of nerve fiber membranes for sodium ions, thereby reducing the spread of impulses from the focus of epileptic activity.

    The effect occurs 20-60 minutes after oral administration.

    Pharmacokinetics:

    Benzobarbital is metabolized with the formation of an active metabolite - phenobarbital, which in turn has an antiepileptic effect.

    Binding to blood plasma proteins in the drug is weak. High concentrations of the drug can be found in the brain, kidneys and liver.The drug has the ability to penetrate through the histohematological barriers. Benzobarbital excreted in breast milk. Half-life from the body is 3-4 days.

    It is excreted by the kidneys both in unchanged form and in the form of metabolites.

    Indications:

    Epilepsy (of various origins), generalized and partial seizures.

    Contraindications:

    Severe violations of kidney and / or liver function, respiratory failure, diabetes mellitus, adrenal insufficiency, hyperkinesia, depressive conditions (with suicidal attempts), chronic heart failure II-III stage, porphyria, anemia, bronchial asthma, hypersensitivity to benzobarbital.

    Carefully:

    Elderly age.

    Pregnancy and lactation:

    Contraindications to use during pregnancy and lactation.

    Dosing and Administration:

    The dosage regimen is set individually. Inside - 100 mg 3 times a day.

    Maximum doses: single dose - 300 mg, daily - 800 mg. Treatment begins with a single dose of a single dose. After 2-3 days gradually increase the dose to achieve a clinical effect (reduced frequency or complete cessation of seizures).Treatment is long.

    Children aged 3-6 years - 25-50 mg per reception (100-150 mg per day), 7-10 years - 50-100 mg per reception (150-300 mg per day), 11-14 years - on 100 mg on reception (300-400 mg a day). Maximum doses for older children: single dose - 150 mg, daily - 450 mg.

    Side effects:

    On the part of the hematopoiesis system: thrombocytopenia, anemia.

    From the cardiovascular system: lowering of arterial pressure, thrombophlebitis.

    From the side of the central nervous system: rarely - drowsiness, lethargy, inhibition, headache, ataxia, nystagmus, speech difficulties (in these cases, dose adjustment or caffeine administration is required).

    Other: deterioration of appetite, allergic reactions, bronchospasm.

    Overdose:

    Symptoms: ataxia, athetoid limb movements and hyperkinesis, dysmetry, hyper and hyporeflexia, motor anxiety, convulsions, muscle twitching, trembling, dizziness, drowsiness, mydriasis, tachycardia, hyper- or hypotension, opisthotonus, respiratory depression, shock, oliguria, anuria .

    Treatment is symptomatic.

    Interaction:

    Strengthening the effects of opioid analgesics, anesthetics, antipsychotics, anxiolytics, tricyclic antidepressants, ethanol, hypnotics.

    With simultaneous use, a decrease in the effectiveness of paracetamol, anticoagulants, tetracyclines, griseofulvin, glucocorticosteroids, mineralocorticoids, cardiac glycosides, quinidine, vitamin D, xanthines is observed.

    With simultaneous use with drugs that have a mielodepressivnoe effect, there is an increase in hematotoxicity.

    Special instructions:

    In patients who have previously taken barbiturates, when switching to benzobarbital treatment, sleep disturbances are possible. In these cases, appoint for the night phenobarbital (50-100 mg) or other hypnotics.
