    A.01.B.A.53   Phenylephrine in combination with other drugs

    Pharmacodynamics:
    A combined preparation containing phenylephrine and dimethindene. Provides anticongestive, anti-allergic, vasoconstrictive effect.
    Phenylephrine is a sympathomimetic agent, with local application has a moderate vasoconstrictive effect (due to stimulation of alpha1-adrenoreceptors located in the venous vessels of the nasal mucosa), eliminates the swelling of the nasal mucosa and its paranasal sinuses.
    Dimetinden is an antiallergic agent, a histamine H antagonist1-receptors; does not reduce the activity of the ciliated epithelium of the nasal mucosa.
    Pharmacokinetics:The drug is intended for topical application, its activity does not depend on the concentration of active substances in the blood plasma.
    Indications:
    Acute rhinitis (including runny nose with colds); allergic rhinitis (including hay fever); vasomotor rhinitis; chronic rhinitis; acute and chronic sinusitis; acute otitis media (as anan auxiliary method of treatment); preparation for surgical interventions in the nose area and elimination of edema of the nasal mucosa and accessory sinuses after surgical interventions in this area.

    Contraindications:
    Common for all dosage forms: hypersensitivity to the components of the drug; atrophic rhinitis (including with fetid detachable - ozena); administration of inhibitors monoamine oxidase (simultaneously or in the previous 14 days).
    For the spray and gel nasal additionally: children's age (up to 6 years).
    Carefully:Cardiovascular diseases (arterial hypertension, arrhythmias, generalized atherosclerosis), thyroid disease, angle-closure glaucoma, prostate adenoma, insulin-dependent diabetes mellitus. As with any local vasoconstrictor, caution should be exercised in appointing patients with severe reactions to sympathomimetics (for example, with the development of insomnia and dizziness).
    Pregnancy and lactation:Given the possible systemic vasoconstrictive effect of phenylephrine, the drug is not recommended for use during pregnancy and lactation.
    Dosing and Administration:
    Intranasally, before administration, the nasal passages should be cleared.
    The drops are nasal. Adults and children over 6 years of age - 3-4 drops in each nasal passage 3-4 times a day. Children from 1 year to 6 years - only in the form of drops nasal 1-2 drops in each nasal passage 3-4 times a day: from 1 year to 6 years - 1-2 drops, up to 1 year - 1 drop in each nasal passage 3-4 times a day. Bury intranasally, head thrown back. This position of the head is kept for a few minutes.
    Spray nasal. Adults and children over 6 years of age are prescribed 1-2 injections in each nasal passage 3-4 times a day. When using spray sprayer should be kept vertically, tipped to the top. Holding the head straight, insert the tip into the nasal passage, once with a short sharp movement squeeze the nebulizer, and taking the tip out of the nose, unclench. It is recommended to inhale slightly through the nose during the injection.
    Gel nasal. Adults and children over 6 years of age are applied to each nasal passage as deep as possible 3-4 times a day (the last application is performed shortly before sleep).The use of the drug immediately before bedtime provides an effect throughout the night.
    Side effects:Rarely - mild and transitory local reactions from the mucous membrane of the nose (burning sensation or dryness).
    Overdose:
    Symptoms: In case of accidental ingestion by small children, no serious side effects were noted. In most cases, the symptoms of an overdose were absent, but sometimes symptoms of fatigue, pain in the stomach, minor tachycardia, hypertension, agitation, insomnia, pallor of the skin (often in children with accidental ingestion) were sometimes reported.
    Treatment: the appointment of activated carbon and laxatives (gastric lavage is not required); or the introduction of a large amount of fluid (children over 6 years and adults). There is no specific antidote.
    Interaction:
    Phenylephrine (as well as other vasoconstrictors) is contraindicated in patients receiving inhibitors monoamine oxidase at this time or received them within the previous two weeks.
    Do not prescribe the drug simultaneously with tricyclic antidepressants, beta-blockers.
    Special instructions:
    Do not use the drug for more than 1 week without consulting your doctor. Long (more than 2 weeks) or excessive use of the drug can cause tachyphylaxis and ricochet effect (drug rhinitis), lead to the development of systemic vasoconstrictive action.
    Do not exceed the recommended dose of the drug. Otherwise, the development of manifestations of the systemic effect of the drug is possible.
    Children under the age of 6 years only use drops in the nose.
    The preparation in the form of a gel is especially indicated in case of dryness of the nasal mucosa, in the presence of crusts, with the consequences of a nose injury, and also to prevent nasal congestion at night.
    The drug does not have a sedative effect (does not affect the rate of psychomotor reactions).
    Does not affect the ability to drive vehicles and control mechanisms.
