    Pharmacodynamics:

    Dopamine is a cardiotonic, hypertensive drug. Stimulates β1adrenoreceptors, is a dopamine receptor agonist.In high doses, it stimulates α-adrenoreceptors and increases peripheral vascular resistance and systolic blood pressure, increases cardiac contractions, and increases cardiac output. Expands mesenteric and renal vessels, reduces the resistance of the latter, increases blood flow in them, stimulates renal filtration, raises sodium nares. Inhibits the synthesis of aldosterone.

    Pharmacokinetics:

    After intravenous injection is completely distributed in the body, partially passes through the blood-brain barrier. Metabolized in the liver, kidneys and blood plasma with the participation of monoamine oxidase and catechol-O-methyltransferase to inactive metabolites. Half-life of plasma - about 2 minutes, from the body - about 9 minutes. It is excreted by the kidneys: about 80% within 24 hours, mainly in the form of metabolites, a small part of the dose is excreted unchanged.

    Indications:

    - Shock of different etiology (cardiogenic, traumatic, postoperative, etc.);

    - to improve hemodynamic parameters in acute cardiac and vascular insufficiency, the syndrome of "low cardiac output" associated with surgical intervention,especially with surgical operations on the heart.

    Contraindications:

    - Thyrotoxicosis;

    - the closed-angle form of glaucoma;

    benign prostatic hyperplasia;

    - pheochromocytoma;

    - should not be prescribed for arrhythmia in combination with monoamine oxidase inhibitors, with cyclopropane and halogen containing anesthetics.

    Pregnancy and lactation:During pregnancy and during lactation, Dopamine-Ferein® apply only in cases where the intended benefit to the mother exceeds the potential risk to the fetus or child.
    Dosing and Administration:

    Enter intravenously drip slowly.

    The content of the ampoule (0.5% and 4% solution of dopamine) is diluted respectively in 125 or 400 ml of a 5% solution of glucose or isotonic sodium chloride solution, so that 1 ml of the solution contains 200 or 500 μg of dopamine, respectively. If contraindications to the introduction of large volumes of fluid apply more concentrated solutions, where 1 ml contains 800 mcg. Infusions are carried out continuously for 2-3 hours to 1-4 days or more. The average daily dose is 400-800 micrograms. The effect of the drug comes quickly and ends after 5-10 minutes after the end of the injection.

    Dopamine-Ferein® administered under cardiac monitoring. Doses and the rate of administration are determined taking into account the level of arterial pressure, heart rate, heart rate, cardiac output and diuresis.

    Side effects:

    From the side of the cardiovascular system: tachycardia, chest pain, increased blood pressure; when using high doses, arrhythmia.

    From the gastrointestinal tract: possible nausea, vomiting.

    From the central nervous system: headache, anxiety, tremor.

    Allergic reactions: possible bronhospazm.

    Local reactions: when dopamine enters the skin, necrosis of the skin and subcutaneous tissue is possible.

    Interaction:

    With the simultaneous use of Dopamine-Ferein® and octadine increased sympathomimetic effect; with monoamine oxidase inhibitors - it is possible to intensify the intensity and duration of the cardiostimulating and pressor effect of dopamine.

    The introduction of Dopamine-Ferein® on the background of the administration of tricyclic antidepressants leads to an increase in its effects (possibly the development of tachycardia, arrhythmia, severe arterial hypertension).

    Diuretics enhance the diuretic effect of Dopamine-Ferein®.
