    Pharmacotherapeutic group:vasodilating agent-nitrate
    ATX: & nbsp

    C.01.D.A.05   Pentaerythrityl tetranitrate

    Pharmacodynamics:

The venous vasodilator has an antianginal effect. Stimulates the formation in the wall of vessels of nitric oxide, which is an endothelial relaxing factor. Reduces preload (widening of the peripheral veins and a decrease in the flow of blood to the right atrium) and postnagruzka common peripheral vascular resistance (OPSS) on the heart; reduces the need for myocarda in oxygen.

    Restores blood flow through small endocardial vessels, improves blood flow in the epicardial coronary arteries, increases exercise tolerance in patients with ischemic heart disease (CHD),angina pectoris;

With heart failure helps discharge the myocardium by reducing preload. Reduces pressure in a small circle of blood circulation. Compared with nitroglycerin, it has a slower but longer action (up to 10h). When administered orally, the onset of the effect is after 30-45 minutes (with sublingual - after 15-20 minutes), duration - 4-5 hours.

    Pharmacokinetics:

    Slowly absorbed by mucous membranes.

Indications:Erynitol is used to prevent attacks of angina pectoris.
    Contraindications:

Hypersensitivity, severe arterial hypotension (systolic blood pressure below 90 mm Hg, diastolic arterial pressure below 60 mm Hg), collapse, shock, acute myocardial infarction, flowing with hypotension, hemorrhagic stroke, craniocerebral trauma, occlusive glaucoma, severe hepatic insufficiency (risk of methaemoglobinaemia), severe renal failure, severe anemia, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, severe aortic stenosis and / or mitral valve, hyperthyroidism, primary pulmonary hypertension (hyperemia of insufficiently ventilated alveolar zones can lead to hypoxia), chronic heart failure with low left ventricular filling pressure, simultaneous administration of phosphodiesterase-5 inhibitors (incl. sildenafil, vardenafil, tadalafil), intolerance to lactose and sucrose, deficiency of sucrose-isomaltase, lactase or glucose-galactose, malabsorption, hypersensitivity to the drug and its components, other organic nitrates, age to 18 years (efficacy and safety not established).

Carefully:Applied with a tendency to arterial hypotension in the elderly, dehydrated patients; with an inclination to orthostatic hypotension in the anamnesis.
Pregnancy and lactation:The use of the drug during pregnancy and during lactation is contraindicated.
    Dosing and Administration:

Erynitum taken inside (1 hour before meals) in the form of tablets of 10-20 mg (1-2 tablets) 3-4 times a day; if necessary, up to 40 mg 4 times a day; an appointment of 80 mg 2 times a day is acceptable.

To prevent night attacks, angina pectoris is prescribed 20 mg at bedtime.

The course of treatment usually lasts 2-4 weeks. The repeated course is prescribed with interruptions, the duration of which depends on the course of the disease.

If necessary, sublingual administration of the drug is possible, but not for arresting an attack of angina pectoris.

Side effects:Allergic reactions, skin hyperemia, headache, tinnitus, dizziness, nausea, weakness, tachycardia, increased fatigue, anxiety, increased sweating, lowering blood pressure (BP), orthostatic hypotension; sometimes there is a disorder of the bowel function (diarrhea). When the dose is reduced or after discontinuation of the drug, side effects occur.
    Overdose:

When using the drug in large doses, the development of tolerance is possible.

Symptoms: Orthostatic hypotension, sensation of head compression, throbbing headache, severe sweating, cold skin, covered with sticky sweat, or hyperemic, palpitations, bradycardia, conduction blockade, shortness of breath, Kussmaul breathing, bloody diarrhea, vomiting, dizziness, confusion , syncopal conditions, coma, fever, paralysis, visual impairment.

Treatment: gastric lavage, imparting a horizontal position with the raised feet, with severe arterial hypotension and / or in patients in shock - introduction of fluid into / from a drip infusion solutions, monitoring the level of methemoglobin in the blood by its increasing - assignment of ascorbic acid at a dose of 1 g of inside, methylthioninium chloride (methylene blue) / in 50 ml of a 1% solution of toluidine blue - 2-4 mg / kg body weight / in quickly, if necessary - again at a dose of 2 mg / kg with intervals of 1 hour, oxygen , hemodialysis, exchange swings vanie blood.
    Interaction:

If you are taking this medication with other medicines at the same time, talk to your doctor.

Under the influence of dihydroergotamine, the antianginal effect of erinite can be reduced. The combination of these funds is considered irrational.

Perhaps greater hypotensive action while applying a vasodilator and antihypertensive agents (beta-blockers, calcium channel blockers, etc.), Antipsychotics, tricyclic antidepressants, monoamine oxidase inhibitors, procainamide, ethanol, 5-phosphodiesterase inhibitors.

Combination with anaprilin is rational, since the antianginal effect is enhanced.

    Special instructions:

At reception raises risk of development of an arterial hypotension at elderly patients, dehydrated patients.

It is necessary to exclude alcohol during treatment. When prolonged use may reduce the therapeutic effect (due to the development of tolerance). To reduce the risk of developing tolerance, it is recommended to observe the "non-nitrate" period (8-12 hours).

When using the drug once a day in combination with other antianginal drugs, the risk of developing tolerance is less.

The drug is not used to stop angina attacks. Do not suddenly interrupt treatment with the drug.

Effect on the ability to drive transp. cf. and fur:

Due to the fact that the use of the drug may cause such side effects as dizziness, drowsiness and blurred vision, so do notIt is necessary to refrain from driving vehicles and practicing potentially dangerous activities that require increased concentration of attention and rapid mental and motor reactions.
