  • Pharmacodynamics:

    Guar gum is a dietary fiber obtained from seeds Cyamopsis Tetragonolobus, plants of the legume family. Guar gum is a polysaccharide that forms a viscous gel in water.

    Guar gum slows down the emptying of the stomach, as well as the absorption of carbohydrates in the small intestine. This reduces the level of glucose in the blood.

    Reduction in cholesterol in the plasma is provided by a decrease in the absorption of cholesterol, an increase in the secretion of bile acids, and the activation of the metabolism of cholesterol in the liver. Thus, the action of guar gum appears to be similar to that of ion-exchange resins that bind bile acids. With monotherapy, guar gum reduces the levels of total cholesterol and low-density lipoprotein cholesterol in plasma by approximately 10-15%.

    Pharmacokinetics:

    Guar gum in the gastrointestinal tract is not absorbed. In the gastrointestinal tract, guar gum is partially destroyed by bacteria.

    Indications:

    Diabetes mellitus type 2 with concomitant obesity, with ineffectiveness of dietary measures (as part of complex therapy).

    Hypercholesterolemia, with ineffectiveness of dietary measures.

    Overweight, obesity.

    Contraindications:

    Hypersensitivity to guar gum.

    Obstructive or stenotic lesions of the gastrointestinal tract.

    Pregnancy and lactation:

    Since guar gum in the gastrointestinal tract is not absorbed, there are no contraindications for its use during pregnancy and breastfeeding.

    Dosing and Administration:

    The drug is taken orally.

    Diabetes mellitus type 2: 1 sachet (5 g) or 2 sachets (2.5 g) 3 times daily with meals.

    Hypercholesterolemia: 1 sachet (5 g) or 2 sachets (2.5 g) 2-5 times a day during meals.

    Excess body weight: 1 sachet (5 g) or 2 sachets (2.5 g) 3 times daily with meals.

    It is preferable to start therapy with Guar medication with a low dose, for example 2.5 g, since in this way it is possible to avoid the development of gastrointestinal side effects caused by the drug.

    Guar in granules should be taken either with food, or with a liquid (water, juice, milk) with meals.

    Dry granules Guarem should not be taken without food. One dose of granules should be drunk at least one glass of liquid.

    Side effects:

    The side effects of guar gum are mostly light and transient.

    Side effects are limited mainly to the digestive tract. Most often, flatulence occurs, developing in approximately 50% of cases. In addition, abdominal pain, nausea, and diarrhea are possible.

    Guar gum very rarely causes obstruction of the esophagus or small intestine, and approximately 50% of such cases have a history of esophageal or intestinal disease.

    Inhalation of dust containing guar gum can cause allergic rhinitis and nocturnal apnea.

    Overdose:

    Cases of an overdose of guar gum are not described.

    Taking high doses of the drug with a high probability can cause violations of the gastrointestinal tract; should take into account the risk of obstructive bowel disease.

    Interaction:

    Guar gum can slow absorption of a number of drugs from the intestine.However, no significant changes in the bioavailability of other drugs have been observed. The slowing down of absorption of paracetamol, digoxin, buumetadine has been revealed. It is believed that this interaction has little clinical relevance; At the same time, a decrease in the bioavailability of phenoxymethylpenicillin, metformin, glibenclamide, oral contraceptives with simultaneous application with guar gum is described. Therefore, the use of guar gum with these preparations should be avoided.

    Special instructions:

    The risk of obstructive lesions of the gastrointestinal tract caused by the use of guar gum may increase if there is a history of abnormalities in the function of the esophagus, stomach or intestines.

    At the beginning of the application of guar gum on the background of therapy with insulin or sulfonylurea derivatives, an increase in the risk of developing hypoglycemia is possible.

    Effect on the ability to drive transp. cf. and fur:

    Guar gum does not affect the ability to drive vehicles and other mechanisms. However, the development of hypoglycemia against the background of the use of guar gum incombination with other hypoglycemic drugs can lead to impaired ability to drive vehicles and perform other actions requiring attention. In patients with diabetes mellitus with good glycemic control, the drug does not affect the ability to drive vehicles and other mechanisms.
