    Pharmacodynamics:

    Combined drug, has antiseptic, disinfectant and local anesthetic effect.

    Boric acid has antiseptic, fungistatic action. Has a slight irritant effect on granulation tissue.

    Procain - a local anesthetic with moderate anesthetic activity and a large breadth of therapeutic effect. Being a weak base, it blocks sodium channels, prevents the generation of pulses in the endings of the sensory nerves and impulses along nerve fibers. Changes the action potential in the membranes of nerve cells without a pronounced effect on the resting potential.Suppresses carrying out not only pain, but also impulses of another modality.

    Pharmacokinetics:

    Boric acid absorbed through damaged skin, wound surface, mucous membranes. In young children, especially well penetrates through the skin and mucous membranes (described as fatal outcomes with accidental ingestion or application to the damaged skin). It is withdrawn slowly (with repeated injections cumulates). About 50% is excreted in urine for 12 hours, the remainder - within 5-7 days.

    Procain is subjected to complete systemic absorption. The degree of absorption depends on the site and route of administration (especially on vascularization and blood flow velocity in the area of ​​administration) and the final dose (amount and concentration). It is rapidly hydrolyzed by plasma and liver esterases with the formation of two major pharmacologically active metabolites: diethylaminoethanol (has a moderate vasodilating effect) and paraaminobenzoic acid (PABA) (a competitive antagonist of sulfanilamide chemotherapeutic drugs and can weaken their antimicrobial effect). The half-life is 30-50 days, in the neonatal period - 54-114 days.It is excreted mainly by kidneys in the form of metabolites, in unchanged form it is deduced no more than 2%.

    Indications:External acute and chronic otitis without damage to the tympanic membrane.
    Contraindications:

    Hypersensitivity to the components of the drug, individual intolerance, pregnancy, the period of breastfeeding, children under 1 year old, chronic renal failure, violation of the integrity of the tympanic membrane.

    Pregnancy and lactation:

    The use of the drug in pregnancy and lactation is contraindicated.

    Dosing and Administration:

    In the ear canal.

    Adults 2-3 drops in the ear 2-3 times a day.

    Duration of therapy: not more than 7 days.

    Children 1-2 drops 2-3 times a day. It is used in the form of tampons, moistened with a solution, which is injected into the ear canal.

    Before using ear drops, it is recommended to warm the bottle holding it in your hand, in order to avoid the unpleasant sensations associated with the ingress of cold liquid into the ear. Drops should be instilled in the patient lying with his ear turned upwards; After the procedure the patient should remain in this position for several minutes.

    Side effects:

    Allergic reactions.Skin rash, desquamation of the epithelium, confusion, convulsions, ooligia, rarely anaphylactic shock.

    With prolonged use, there may be headache, nausea, vomiting, diarrhea, in rare cases, shock.

    When using the drug, local reactions are possible in the form of itching, burning, which pass independently after 1-2 minutes and do not require drug withdrawal.

    Toxic reactions in the case of perforated tympanic membrane: nausea, vomiting, reddening of the skin, itching, urticaria, edema, deafness, imbalance, confusion.

    Overdose:

    In case of an overdose, prolonged use and in case of violations of kidney function, acute and chronic toxic reactions may arise: nausea, vomiting, diarrhea, skin rash, headache, confusion, urinary disorders, in rare cases, shock.

    Treatment: symptomatic.

    Interaction:

    The metabolite of procaine (para-aminobenzoic acid) is a sulfonamide antagonist.

    Special instructions:

    Avoid contact with mucous membranes.

    Before using the drug, you need to ensure the integrity of the tympanic membrane.In case of application with a perforated tympanic membrane, the drug can come into contact with the organs of the middle ear and lead to the appearance of toxic reactions.
