    Pharmacodynamics:

    Structural analogue of PABA, necessary for microorganisms for the synthesis of dihydrofolic acid. Competitive blockade of dihydropteroate synthetase of bacteria: disruption of the synthesis of dihydropteroic acid (precursor of tetrahydrofolic acid - cofactor of purine synthesis). Only those microorganisms that are forced to synthesize folic acid themselves are sensitive.

    Pharmacokinetics:

    Absorption is fast. Distribution is good, including in cerebrospinal fluid. The connection with plasma proteins is 75-86%. Biotransformation in the liver (acetylation). Half-life 7 hours. Elimination by the kidneys.

    Indications:

    Infectious-inflammatory diseases caused by susceptible to malaria infections: tonsillitis, sinusitis, otitis media, exacerbation of chronic bronchitis, pneumonia, inflammatory diseases of the biliary and urinary tract infections, shigellosis, erysipelas, wound infection.

    Contraindications:

    Hypersensitivity, oppression of bone marrow hematopoiesis, hyperbilirubinemia in children (risk of bilirubin encephalopathy), congenital deficiency G-6-PD, porphyria, azotemia, pregnancy, breast-feeding.

    Carefully:

    Allergy to other medicines containing a sulfonamide moiety; CRF, hepatic insufficiency, bronchial asthma, systemic lupus erythematosus, G-6-FD deficiency, age up to 2 months, HIV infection, AIDS (systemic use).

    Pregnancy and lactation:

    Adequate and well-controlled studies in humans and animals have not been conducted.Do not apply! There is no information on the penetration into breast milk. Do not apply!

    Dosing and Administration:

    Is taken internally.

    Pneumonia:

    For adults - for 1 reception appoint 2 g, then 1 g 4-6 times a day.

    For children older than 3 years - 0.1 g / kg for the first reception, then for 0.1-0.15 g / kg / day, divided into 4-6 receptions.

    Acceptance of the drug is stopped 2-3 days after the disappearance of the symptoms of the disease.

    Tonsillitis:

    For adults - 1 g 2-3 times a day;

    For children older than 3 years - 0.05-0.075 g / kg / day, divided into 2-3 admission.

    Duration of therapy: 5-7 days.

    Sinusitis:

    For adults - 1 g 4-6 times a day;

    For children older than 3 years - 0.1-0.15 g / kg / day, divided into 4-6 receptions.

    Duration of therapy: 7-10 days.

    Otitis media:

    For adults - 1 g 2 times a day;

    For children older than 3 years - 0.05-0.075 g / kg / day, divided into 2 doses.

    Duration of therapy: 7-10 days.

    Exacerbation of chronic bronchitis:

    For adults - 1 g 4-6 times a day;

    For children older than 3 years - 0.1-0.15 g / kg / day, divided into 4-6 receptions.

    Duration of therapy: 10-14 days.

    Inflammatory diseases of the bile and urinary tract:

    For adults - for 0.5 g 4-6 times a day;

    For children older than 3 years - 0.05-0.075 g / kg / day, divided into 4-6 receptions.

    Duration of therapy: 7-10 days.

    Shigellosis: Take 2 courses of treatment.

    The first course: 1 and 2 days - 1 g every 4 hours (6 g / day total), 3 and 4 days - 1 g every 6 hours (4 g / day), 5 and 6 days - 1 g every 8 h (3 g / day).

    After 5-6 days, the second course is carried out: 1 and 2 days - 1 g every 4 hours, at night - 8 hours (5 g / day total), 3 and 4 days - 1 g after 4 hours (4 g / day, at night do not give), day 5 - 3 g / day.

    Iroja:

    For adults, 1 g 4-6 times a day.

    For children older than 3 years - 0.1-0.15 g / kg / day, divided into 4-6 receptions.

    Duration of therapy: 7-10 days.

    Wound infections:

    Higher doses:

    For adults: single - 2 g, daily - 7 g;

    Side effects:

    Hypersensitivity (fever, itching, skin rash, Stevens-Johnson syndrome, toxic epidermal necrolysis, fulminant liver necrosis, agranulocytosis, aplastic anemia, other blood diseases), photosensitization.

    Diseases of the blood (fever, sore throat, pallor, unusual bleeding or bruising, unusual fatigue or weakness).

    Hepatitis, Lyell's syndrome.

    Disorders from the central nervous system: confusion, disorientation, euphoria, hallucinations, depression.

    Colitis caused by Clostridium difficile.

    Crystalluria, hematuria; goiter, other thyroid dysfunction, interstitial nephritis, tubular necrosis.

    Nausea, vomiting.

    Overdose:

    Oppression of bone marrow hematopoiesis: leukopenia, agranulocytosis, infection.

    Treatment is symptomatic.

    Interaction:

    Cimetidine - increases the risk of developing leukopenia.

    Special instructions:

    Used to determine the activity of liver enzymes (the phenotype of acetylation).
