    Pharmacodynamics:Non-selectively suppresses COX1 and COX2, regulating the synthesis of prostaglandins. It indiscriminately suppresses synthetases of prostaglandins. Suppresses the production of anti-inflammatory cytokines. Has analgesic, antipyretic and anti-inflammatory effect.

    Reduces the level of uric acid in the blood.

    Pharmacokinetics:After ingestion, up to 90% is absorbed in the gastrointestinal tract. The maximum concentration in the blood plasma is achieved in 1-2 hours. The connection with plasma proteins is over 95%.

    Penetrates through the blood-brain and placental barrier, is found in breast milk. Metabolism in the liver to two major metabolites.

    The half-life is 18-21 hours. Elimination by the kidneys (70%, of which 5% is unchanged) and with feces (30%) in the form of metabolites.

    Indications:

    It is used for the treatment (systemic use) of inflammatory and degenerative diseases of the musculoskeletal system: rheumatoid arthritis, gouty, psoriatic arthritis, ankylosing spondylitis (Bechterew's disease), osteoarthritis, as part of a complex therapy for systemic lupus erythematosus.Used to relieve the pain syndrome in bursitis, tendovaginitis, arthralgia, myalgia, neuralgia, migraine, dental and headache, algodismenore, pain in trauma, burns, as well as with a febrile syndrome that accompanies "cold" and infectious diseases.

    Used for topical application in articular syndrome during exacerbation of gout, rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, arthralgia, myalgia, neuralgia, phantom pain. It is used for dermatitis due to various chemical and mechanical effects, burns, including sun, inflammatory reactions in places of intramuscular and intravenous injections, insect bites, superficial thrombophlebitis, inflammation of hemorrhoids, traumatic injuries of soft tissues, inflammatory infiltrates, hematoma, muscle stretches and tendon, bursitis, tenosynovitis.

    Contraindications:Recurrent polyposis of nose and paranasal sinuses, bronchial asthma, intolerance to drugs of pyrazolone series and acetylsalicylic acid - "aspirin" triad. Renal and hepatic insufficiency, leukopenia, chronic heart failure, arrhythmias, peptic ulcer of the stomach and duodenum, children under 6 months.

    For topical application: eczema and ulcers with trophic skin lesions.

    Pregnancy and lactation:

    Dosing and Administration:Use in children

    Older than 6 months: 10-100 mg, depending on age, 3-4 times a day.

    Adults

    Inside during or after a meal, 100-150 mg 2-3 times a day.

    The highest daily dose: 600 mg.

    The highest single dose: 150 mg.

    Outer: apply on the surface of the skin a thin layer 2-3 times a day.

    Side effects:

    Central and peripheral nervous system: dizziness, headache, sleep disorders, depression, agitation, hallucinations.

    Respiratory system: reduced resistance to respiratory tract infections, bronchospasm, shortness of breath, cough.

    Hemopoietic system: anemia, agranulocytosis, leukopenia, thrombocytopenia.

    The cardiovascular system: tachycardia, arterial hypertension.

    Digestive system: bleeding gums with prolonged use.

    Dermatological reactions: hyperhidrosis, photosensitivity, alopecia.

    Sense organs: tinnitus, hearing loss.

    Urinary system: edematic syndrome, renal failure.

    Reproductive system: uterine bleeding.

    Overdose:Cyanosis of the lips, skin, fingertips, headache, dizziness, hyperventilation of the lungs,blurred consciousness.

    In children, vomiting, myoclonic seizures, hemorrhage, abdominal pain, impaired kidney and liver function.

    Treatment is symptomatic.

    Interaction:

    Strengthens the effect of anticoagulant drugs.

    Increases the concentration in the blood of lithium preparations, methotrexate.

    Tricyclic antidepressants, antacids and colestramine reduce the absorption of phenylbutazone.

    Reduces the uricosuric effect of acetylsalicylic acid.

    Phenobarbital reduces the concentration of phenylbutazone in plasma.

    Special instructions:

    Control of the picture of peripheral blood and the functional state of the liver and kidneys.

    Ineffective use of ointment with thrombophlebitis of deep veins.

    In the treatment it is not recommended to drive vehicles and work with moving mechanisms.
