    Pharmacodynamics:Inhibits the factor necessary for the translocation of protein subunits and the lengthening of the peptide chain of pathogenic bacteria, which leads to the death of microorganisms. Bacteriostatic acts against Gram-positive bacteria Staphylococcus spp., including most strains S. aureus (including MRSA - methicillin-resistant S. aureus) and S. Epidermidis (including methicillin-resistant), has activity against Corynebacterium spp., Cladosporium spp. Has a low activity in relation to Streptococcus spp., including S. Pneumoniae. Inactive for Salmonella spp., Proteus spp., Escherichia coli.

    Pharmacokinetics:After ingestion, up to 90% is absorbed in the gastrointestinal tract. After absorption into the blood or intravenous administration, the maximum concentration in the blood plasma is reached after 2 hours.It accumulates in pus, sputum, abscesses of the brain, concentrates in bone tissue, sequestrants, synovial fluid, burn scabs.

    Penetrates through the placental barrier, excreted in breast milk. Metabolism in the liver. With prolonged use, cumulates.

    The half-life is 9-14 hours. Elimination with feces in the form of inactive metabolites.

    When topical application penetrates into the intraocular fluid and maintains therapeutic concentration for 12 hours.

    When external application does not have a systemic effect.

    Indications:Inside and intravenously - with septicemia, abscesses of the brain, infections of bones and joints, endocarditis, infectious diseases of the eyes, skin and soft tissues, pneumonia, infectious and inflammatory complications in cystic fibrosis, wound infection.

    Used for external use with impetigo, carbuncles, boils, infected wounds, hydradenitis, folliculitis, erythrasis, paronychia, sycosis of the beard, vulgar acne. For topical application in ophthalmology it is used for infectious and inflammatory diseases of the eyes, also for infections associated with the removal of a foreign body from the conjunctiva and the cornea.

    Contraindications:Hepatic insufficiency, increased content of prothrombin in the blood plasma, individual intolerance.

    Carefully:Period newborns (especially with prematurity), hypersensitivity.

    Pregnancy and lactation:

    Recommendations for FDA - category C. It is used with caution during pregnancy and lactation in those cases,when the intended benefit exceeds the risk to the fetus and newborn. Contraindicated in the III trimester of pregnancy.

    Dosing and Administration:

    Use in children

    Inside after eating as a suspension, 20-80 mg / kg body weight.

    Intravenously drip, 2-3 times a day at a rate of 10-15 drops per minute. The daily dose is 20-40 mg / kg.

    Adults

    Inside after a meal of 0.5-1.0 grams 3 times a day.

    Intravenously drip, 2-3 times a day at a rate of 60-80 drops per minute. The daily dose is 1.5-2.0 g.

    Locally, as an ointment or cream, apply to the affected surface of the skin (after removal of necrotic masses and pus) with burns 2-3 times a week. It is allowed to use a dressing.

    Conjunctival, 1 drop 2 times a day.

    The highest daily dose: 3 g.

    The highest single dose: 1 g.

    Side effects:

    Ingestion: dyspepsia, in infants - regurgitation.

    When administered intravenously: phlebitis and periphlebitis, nausea, vomiting, skin rashes.

    Allergic reactions.

    Overdose:Cases of overdose are not described.

    Treatment is symptomatic.

    Interaction:Weaken the bactericidal effect of penicillins and cephalosporins.

    Pharmacological is incompatible in one solution with vancomycin, gentamicin, kanamycin, carbenicillin, cephaloridine, amino acid solutions, calcium-containing and blood substitute drugs. Precipitation occurs in infusion solutions at pH below 7.4.

    Special instructions:

    Intramuscular injections of the drug are contraindicated because of the pronounced local irritant effect, up to necrosis.
