    Pharmacodynamics:

    Triterpenovy glycoside (saponin) from the fruits (seeds) of horse chestnut (Aesculus hippocastanum L., semen) (in the form of sodium salt). Has a pronounced capillaroprotective activity, has an antiexudative effect.

    Also used as a laxative. Triterpene saponin escin stimulates interoceptors of the rectal mucosa, causing an increase in its peristalsis and rapid evacuation of stool. The effect occurs 5-15 minutes after the application of the suppository. The urge to empty the bowel stops after the act of defecation. The drug does not cause stretching of the walls of the rectum, strengthens and tones up its smooth muscles. With a course application, the drug restores the defecation reflex and normalizes the defecation biorhythm.

    Pharmacokinetics:

    After oral administration of 50 mg of horse chestnut fruit extract, the maximum concentration in blood plasma is achieved after 2.35 hours and is 25.31 ng / ml.

    The binding with plasma proteins is 84%.

    Half-life is 19.87 hours after ingestion of 0.11% of the extract of horse chestnut fruit is excreted by the kidneys in the form of escin.

    With rectal administration, the drug does not have time to enter the bloodstream and therefore does not affect the metabolic processes of the body.

    Indications:

    Post-thrombophlebitic syndrome and some symptoms of chronic venous insufficiency of various etiologies (including edema, cramps in the calf muscles, pain and feeling of heaviness in the legs), varicose veins, trophic disorders, hemorrhoids. Post-traumatic or postoperative edema of soft tissues, trauma, including sports (to accelerate the restoration of the function of organs and tissues).

    Functional constipation of different etiology: habitual, psychogenic, senile, constipation in sedentary patients, as well as constipation associated with a decrease in the sensitivity of the receptor apparatus of the rectum and violation of biorhythm defecation).

    Contraindications:

    Hypersensitivity, chronic renal failure, I trimester of pregnancy, breastfeeding; with external application: violation of the integrity of the skin (wounds, deep cuts, trophic ulcers).

    For suppositories: intestinal obstruction; acute inflammatory diseases of the abdominal cavity; acute hemorrhoids; acute proctitis and paraproctitis; spastic constipation; tumors of the rectum; cracks in the rectum; hypersensitivity to the components of the drug.

    Carefully:

    Propensity to thrombosis, age to 18 years (no experience of use). Simultaneous reception of anticoagulants.

    Pregnancy and lactation:

    Contraindicated in the first trimester of pregnancy. Possible use in the III trimester of pregnancy, especially with the appearance of swelling of the lower extremities and the absence of nephropathy.

    It is not recommended to use during breastfeeding.

    The use of the drug in the form of suppositories during pregnancy and lactation (breastfeeding) is possible only in cases where the potential benefit to the mother exceeds the risk for the fetus and the baby.

    Dosing and Administration:

    Intravenous adults - 5-10 mg per day, children under the age of 3 years - 100 mcg / kg per day, from 3 to 10 years - 200 mcg / kg per day.

    The dose for oral administration depends on the dosage form and is determined individually.

    With external application, rub gently in the affected area several times a day.

    Suppositories. Adults appoint 1 suppository (50-100 mg) once a day; the maximum single and daily dose is 300 mg per day. The duration of treatment should not exceed 30 days.

    The drug is used rectally (injected into the rectum) 1 time per day for 5-15 minutes before the planned act of defecation.

    Side effects:

    Dyspeptic phenomena (including nausea), allergic reactions (sensation of heat, skin rash, itching, urticaria).

    There are reports of the development of acute renal failure in patients who underwent heart surgery who escin was administered intravenously in high doses.

    For suppositories: in some cases, allergic reactions, a brief burning sensation in the rectum.

    Overdose:

    Not described. Treatment is symptomatic.

    Suppositories. Symptoms: irritation of the mucous membrane of the rectum, in some cases - pain in the rectum.

    Interaction:

    With intravenous injection of escin to women taking hormonal contraceptives, the dose of the solvent should be doubled.

    With simultaneous use with escin, the nephrotoxic effect of the antibiotics of the aminoglycoside group is possible.

    With simultaneous use with escin, it is possible to increase the activity of anticoagulants and other drugs with antithrombotic effect.

    With the simultaneous use of cephalosporins displace escin from the connection with plasma proteins.

    Special instructions:

    During treatment, kidney function should be monitored periodically.
