    C.02.A.A.04   The amount of alkaloids Rauwolfia

    Pharmacodynamics:

    A vegetable preparation containing the amount of alkaloids from the roots of rauwolfia (reserpine, serpentine, aymalin and others).

    Has antihypertensive, antiarrhythmic, and sedative effects.

    Pharmacokinetics:After intake of alkaloids rauwolfia well absorbed from the gastrointestinal tract. The maximum concentration of the drug in the blood is detected after 1-3 hours of admission. The connection with plasma proteins is 40-50%, the half-life is 50-170 hours. The drug is metabolized in the liver and intestines. About 8% of the accepted dose is excreted in the urine in the form of metabolites, 62% - intestine in unchanged form.
    Indications:Arterial hypertension I degree of severity.

    Contraindications:
    • Hypersensitivityto the components of the drug
    • Stomach ulcer and duodenal ulcergut
    • Nephrosclerosis
    • Arterial hypotension
    • Insufficiency of coronary circulation
    • Organic damage to the myocardium
    • Aortic defects
    • Depression
    • Pregnancy III term
    • Age under 18 years old
    Carefully:Hypersensitivity to alkaloids rauwolfia.
    Pregnancy and lactation:Contraindicated in pregnancy (III trimester), during lactation.
    Dosing and Administration:

    Inside, after eating, as a rule, at night. On the first day take 1 tablet (2 mg of the amount of alkaloids) at night; on Day 2 - 1 tablet 2 times a day, 3 days - 3 tablets, gradually bringing the total dose to 4-6 tablets per day.

    After reaching a therapeutic effect (after 10-14 days), the dose is gradually reduced to 1-2 tablets per day. The course of treatment is 3-4 weeks. In some cases, according to the indications, the drug is taken for a long time in a maintenance dose of 1 tablet per day.

    Side effects:
    • General weakness, increased fatigue
    • Increased sweating
    • Swelling of the nasal mucosa
    • Cardialgia
    • Bradycardia
    • Depression
    • Low libido
    Overdose:

    Symptoms: severe weakness, sweating, arterial hypotension.

    Treatment: gastric lavage, ingestion of activated carbon, sodium sulfate. With severe hypotension, norepinephrine is prescribed.

    Interaction:When combined with guanethidine, quinidine, beta blockadetori, cardiac glycosides increases the danger of increasing the negative chrono- and dromotropic effects.
    Special instructions:Patients who constantly take the drug should refrain from driving vehicles and practicing potentially dangerous activities that require concentration of attention and speed of psychomotor reactions.
