G.04.B.E.04   Yohimbine

Pharmacodynamics:

Yohimbine is a selective α2adrenoreceptors. Increases the flow of sympathetic impulses from the central nervous system and the release of norepinephrine from the nerve endings, thus providing a sympathomimetic effect. Increases motor activity. It causes vasodilation of the renal, intestinal and genital arteries; stimulates sexual activity. It has been experimentally established that yohimbine normalizes sexual function and coitus ability weakened by stress, eliminates inhibition of erection caused by stimulation of α2-adrenoreceptors, has a stimulating effect on the central nervous system.

The drug is low-toxic, does not affect the genetic apparatus of spermatozoa.

Pharmacokinetics:

Yohimbine is completely absorbed in the digestive tract 45-60 minutes after ingestion. The maximum concentration in the blood is reached approximately 45-75 minutes after administration. In plasma, 82% yohimbine binds to the protein. Evenly distributed in the tissues. Metabolised in the liver and outside it. The half-life of yohimbine after a single dose is 0.25 to 2.5 hours. After 24 hours, yohimbine is practically not detected in the urine. It is excreted mainly by the kidneys. When oral administration of the drug in the recommended doses, cumulation of the drug does not occur.

Indications:

Yohimbine is used as a potentiating agent for treatment of mental and functional forms of impotence.

Contraindications:

- Increased individual sensitivity to the components of the drug;

- arterial hypertension or hypotension;

- severe hepatic / renal insufficiency;

- treatment with adrenomimetics;

- hereditary diseases: galactose intolerance,deficiency of lactase or glucose-galactose malabsorption due to the presence of lactose in the formulation;

- the drug is not intended for children.

Carefully:

Caution should be administered to patients with heart disease (coronary heart disease, tachyarrhythmias), peptic ulcer and duodenal ulcer, mental illness, glaucoma.

Pregnancy and lactation:The drug is not intended for women.
Dosing and Administration:

Assign inside, without chewing, while eating, pouring a glass of water.

The recommended dose is 1-2 tablets 1-3 times a day.

It is advisable to conduct a course of treatment lasting 3-4 weeks. If necessary, the course can be repeated.

The maximum single dose is 2 tablets (10 mg); the maximum daily dose is 6 tablets (30 mg).

Side effects:

Dizziness, headache, anxiety, migraine, arm tremor, vomiting, diarrhea, gastralgia, decreased or increased blood pressure, orthostatic hypotension, tachycardia, priapism.

Overdose:

Symptoms: intoxication manifests general weakness, generalized paresthesia, memory and coordination disorders, severe headache, dizziness,tremor, tachycardia, arterial hypertension and a sense of fear, a significant increase in the level of norepinephrine in the blood plasma, nausea, vomiting, mydriasis, saliva and lacrimation, increased sweating.

Treatment: as an antidote for overdose can be used clonidine, which immediately eliminates the clinical manifestations of intoxication. Assign inside the dose of 0.1-0.2 mg, then again at hourly intervals at a dose of 0.1 mg until complete normalization of the condition. The hypertensive crisis is terminated by the appointment of a combination of a β-adrenergic blocker with an α-adrenoreceptor blocker or a peripheral vasodilator. To eliminate feelings of fear or hallucinations, benzodiazepines are prescribed. Do not prescribe neuroleptics phenothiazine series.

Interaction:

The simultaneous administration of clonidine is not indicated, since drug effects are mutually neutralized.

Yohimbine can alter the activity of antihypertensive agents, potentiate the action of antidepressants and increase the risk of their side effects, reduce the activity of alprozalam.

Clomipramine increases the level of yohimbine in the blood plasma.

During the treatment period, one should refrain from managing motor transport and performing potentially dangerous activities associated with the need for concentration and increased speed of psychomotor reactions.
