  • АТХ:

    G.04.B.E.04   Yohimbine

    Pharmacodynamics:
Presynaptic α2-adrenoceptor, the exact mechanism of action for impotence is not established, it may be associated with blockade of central α2-adrenoceptors and increased sexual desire due to the release of noradrenaline and an increase in the number of excited noradrenergic neurons of the brain; probably, there is also an increased release of norepinephrine in the spongy bodies of the penis; possibly the involvement of other neurotransmitters (dopamine, serotonin) and cholinergic receptors.
Improves erectile function (presumably due to increased blood flow to the penis and reduced outflow). Improves mood, can increase anxiety.
Has antidiuretic effect (possibly due to stimulation of hypothalamic centers and release of the hormone of the posterior lobe of the pituitary gland).
Pharmacokinetics:Quickly and completely absorbed from the digestive tract. Cmax is achieved after 1.3 hours.10 minutes after ingestion, it penetrates the blood-brain barrier and is found in the cerebrospinal fluid, about 17% is detected in the brain tissues for 6 hours. It is metabolized in the liver. The half-life is 0.6 hours.
Indications:Psychogenic and functional impotence, atony of the bladder, climax in men.

V.F50-F59.F52.2   Insufficiency of the genital reaction

XIV.N30-N39.N31   Neuromuscular dysfunction of the bladder, not elsewhere classified

    XIV.N40-N51.N48.4   Impotence of organic origin

Contraindications:Arterial hypertension, ischemic heart disease, liver and / or kidney disease, hypersensitivity to yohimbine, arterial hypotension, simultaneous adrenostimulators.

Side effects:Excitation, tremor, headache, dizziness, anxiety, irritabilityь, high blood pressure, tachycardia, orthostatic collapse (at high doses), increased motor activity, nausea, abdominal pain, vomiting, diarrhea, priapism, decreased diuresis.
    Overdose:

General weakness, generalized paresthesia, memory impairment and movement coordination, headache, dizziness, tremor, tachycardia, increased blood pressure, fear, hallucinations, nausea, vomiting, mydriasis, saliva and lacrimation, increased sweating, increased concentration of norepinephrine in the blood plasma .

Treatment: antidote - clonidine, inside at an initial dose of 0.1-0.2 mg, then again at intervals of 1 hour at a dose of 0.1 mg. Symptomatic treatment: antihypertensive drugs; to eliminate feelings of fear or hallucinations - benzodiazepines (do not prescribe neuroleptics phenothiazine series).

    Interaction:

Antidepressants and normotimics - antagonism is possible.

With simultaneous admission with clonidine - the weakening effect of both drugs.

There may be a decrease in the efficacy of alprazolam.

Hypotensive drugs - antagonism is possible.

Clomipramine increases the concentration of yohimbine hydrochloride in the blood.

    Special instructions:

Do not use for diseases accompanied by a risk of sudden lowering of blood pressure and development of tachycardia.

It is not recommended to combine with antidepressants and other timoleptic drugs.
