  • АТХ:

B.03.A.E.03   Iron preparations in combination with multivitamins

    Pharmacodynamics:

Combined drug. Iron sulfate replenishes the lack of iron in the body, ascorbic acid improves its absorption from the digestive tract.

    Pharmacokinetics:

Absorption and bioavailability of iron - high. Iron is absorbed mainly in the duodenum and the proximal part of the jejunum.

Binding to plasma proteins is 90% or more. Deposited in the form of ferritin or hemosiderin in hepatocytes and cells of the phagocytic macrophage system, a small amount - in the form of myoglobin in the muscles.

    Half-life is 6 hours.

    Indications:

Iron deficiency anemia (treatment and prevention), caused by various causes: as a result of increased iron requirements during pregnancy and lactation, during intensive growth and reconvalescence after long-term serious illnesses; prolonged bleeding (including with peptic ulcer and duodenum guts); decreased iron absorption in chronic diarrhea; inadequate and unbalanced nutrition.

    XXI.Z40-Z54.Z52.0   Blood donor

    III.D50-D53.D50.9   Iron deficiency anemia, unspecified

    XV.O20-O29.O25   Malnutrition in pregnancy

    IV.E50-E64.E61.1   Insufficiency of iron

XV.O95-O99.O99.0   Anemia complicating pregnancy, childbirth and the puerperium

    Contraindications:

Hypersensitivity, elevated iron content in the body (hemosiderosis, hemochromatosis), impaired iron utilization (aplastic anemia), condition after gastrectomy, bleeding; children's age (up to 6 or up to 12 years depending on the manufacturer).

    Carefully:

Stomach ulcer and duodenum guts, inflammatory bowel diseases (enteritis, diverticulitis, ulcerative colitis, Crohn's disease).

Possible use of the drug during pregnancy and lactation according to indications.

    Dosing and Administration:

Dragee and tablets are taken orally (without chewing), 30 minutes before eating, drinking 100 ml of liquid. After the normalization of hemoglobin, treatment is carried out for 1-3 months until the saturation of the iron depot in the body is complete.

The usual dose of the drug in terms of elementary iron for adults is 100-200 mg per day for treatment, 100 mg per day for prevention and maintenance therapy. Children - 3 mg / kg per day.

    Side effects:

Nausea, vomiting, constipation or diarrhea, gastralgia, decreased appetite, skin allergic reactions.

    Overdose:

Symptoms: abdominal pain, vomiting and diarrhea with blood, fatigue or weakness, hyperthermia, paresthesia, pale skin, cold sticky sweat, acidosis, weak pulse, lowering of blood pressure, palpitations. In severe overdose, signs of peripheral circulatory collapse, coagulopathy, hyperthermia, hypoglycemia, liver damage, renal failure, muscle cramps and coma may appear after 6-12 hours.

Treatment: In case of an overdose, immediately consult a doctor. It is necessary to wash the stomach, inside - a raw egg, milk (to bind iron ions in the digestive tract); introduce deferoxamine. Symptomatic therapy.

    Interaction:

Mutually reduce absorption when combined use of tetracyclines, D-penicillamine, tea, egg yolk.

Antacids containing Al salts3+, Mg2+ and Ca2+, reduce the absorption of iron preparations.

Avoid simultaneous prescription with other iron-containing drugs (risk of overdose).

The drug can reduce the absorption of simultaneously used enoxacin, clodronate, grepafloxacin, levofloxacin, levodopa, methyldopa and thyroid hormones.

Do not combine with the following drugs: ciprofloxacin, doxycycline, norfloxacin and ofloxacin.

    Special instructions:

Effective only with iron deficiency anemia and ineffective in anemia of other etiology.

Staining of feces in a dark (black) color against the background of taking iron-containing preparations has no clinical significance.
