    ATX: & nbsp

    N.02.B.A.51   Acetylsalicylic acid in combination with other drugs, excluding psycholeptics

    Pharmacodynamics:

    Combined drug, the effect of which is determined by the components that make up its composition.

    Acetylsalicylic acid has antipyretic, analgesic and anti-inflammatory effects, and also reduces platelet aggregation.

    Antiaggregate effect persists for 7 days after a single dose (more pronounced in men than in women). Increases fibrinolytic activity of plasma and reduces the concentration of vitamin K-dependent clotting factors. It increases hemorrhagic complications during surgical interventions, increases the risk of bleeding on the background of therapy with anticoagulants.

    Stimulates the excretion of uric acid (violates its reabsorption in the renal tubules), but in high doses.

    In the body, it circulates and is distributed in tissues as an anion of salicylic acid.

    Salicylates easily penetrate into many tissues and body fluids. Salicylic acid is metabolized primarily in the liver, is excreted primarily through active secretion in the renal tubules in the form of salicylic acid proper and its metabolites.

    Ascorbic acid allows to satisfy the increased needs of the body in vitamin C for diseases accompanied by increased temperature, and reduces vascular permeability. Ascorbic acid plays an important role in the regulation of oxidation-reduction processes, carbohydrate metabolism,blood coagulability, tissue regeneration, contributes to the body's resistance.

    Indications:Pain syndrome of different genesis in adults (headache, toothache, migraine, neuralgia, muscle pain, menstruation pain). Elevated body temperature for colds and other infectious and inflammatory diseases (in adults and children over 16 years old).
    Contraindications:

    - Erosive-ulcerative lesions of the gastrointestinal tract in the phase of exacerbation;

    - gastrointestinal bleeding;

    - combined use with methotrexate at a dose of 15 mg per week or more;

    - asthma induced by the intake of salicylates or other NSAIDs;

    - the presence in the anamnesis of indications for hives, rhinitis caused by the intake of acetylsalicylic acid and other non-steroidal anti-inflammatory drugs;

    - hemophilia;

    - hemorrhagic diathesis;

    - hypothrombinemia;

    - avitaminosis K;

    - severe renal dysfunction;

    - pregnancy (I and III trimester);

    - lactation period;

    - hypersensitivity to acetylsalicylic acid and other salicylates;

    - deficiency of glucose-6-phosphate dehydrogenase;

    - Children's age (under 16 years) - the risk of developing Reye's syndrome in children with hyperthermia on the background of viral diseases.

    Carefully:

    With liver and kidney disease, with bronchial asthma, erosive and ulcerative lesions and bleeding from the gastrointestinal tract in an anamnesis, with increased bleeding or with simultaneous anticoagulant therapy. Acetylsalicylic acid even in small doses reduces the excretion of uric acid from the body, which can cause an acute attack of gout in predisposed patients. When performing long-term therapy and / or using acetylsalicylic acid in high doses, it is necessary to observe the doctor and regularly check the level of hemoglobin.

    With concomitant therapy with anticoagulants, gout, peptic ulcer and / or duodenal ulcer (in anamnesis), erosive gastritis, and susceptibility to gastrointestinal bleeding, hypoprothrombinemia, hypovitaminosis K, anemia, conditions predisposing to fluid retention in the body (v. h. impaired cardiac function, arterial hypertension), thyrotoxicosis.

    If you have any of these diseases, consult a doctor before taking the drug.

    Pregnancy and lactation:

    Contraindicated in the I and III trimester of pregnancy.In the second trimester of pregnancy, one-time intake of the drug is possible according to strict indications. Acetylsalicylic acid is excreted in breast milk that increases the risk of bleeding in the child due to impaired platelet function, therefore, with prolonged admission during breastfeeding should resolve the issue of ending breastfeeding.

    Dosing and Administration:Is taken internally. Preliminary, the tablet is dissolved in a beaker of drinking water (200 ml) at room temperature. With pain and fever, a single dose of 1 - 2 tablets every 4-6 hours, the maximum daily dose should not exceed 6 tablets. The duration of treatment (without consulting a doctor) should not exceed 5 days with an appointment as an anesthetic and more than 3 days - as an antipyretic.
    Side effects:

    From the gastrointestinal tract: nausea, vomiting, decreased appetite, epigastric pain, diarrhea; rarely - the emergence of erosive and ulcerative lesions, bleeding in the gastrointestinal tract, impaired liver function and exhaustion of the insular pancreas apparatus.

    From the side of the central nervous system: with prolonged use, dizziness, headache, reversible vision disorders, tinnitus are possible.

    On the part of the hematopoiesis system: rarely - thrombocytopenia, anemia.

    From the side of the blood coagulation system: rarely - hemorrhagic syndrome, prolonged bleeding time.

    From the urinary system: when used in high doses - hyperoxaluria and the formation of urinary stones from calcium oxalate, damage to the glomerular apparatus of the kidneys.

    Allergic reactions: rarely - skin rash, swelling Quincke, bronchospasm. When such symptoms appear, it is recommended to stop taking the drug and immediately consult a doctor.

    Overdose:

    In the initial stage of poisoning - excitation of the central nervous system, dizziness, severe headache, reduced hearing acuity, visual impairment, nausea, vomiting, increased breathing. Later there comes drowsiness, convulsions, anuria, depression of consciousness down to coma, respiratory insufficiency, disturbances of water-electrolyte metabolism.

    Treatment: with the appearance of the first symptoms of an overdose - withdrawal of the drug; in case of severe overdose - gastric lavage, appoint Activated carbon and laxative and seek medical attention. Treatment should be carried out in a specialized department.

    Interaction:

    Acetylsalicylic acid enhances the action of other non-steroidal anti-inflammatory drugs, heparin, indirect anticoagulants, uricosuric drugs (probenecid, benzbromarone, sulfinpyrazone), diuretics, antihypertensive drugs, oral hypoglycemic agents, methotrexate. Simultaneous reception with glucocorticoids increases the risk of erosive and ulcerative gastrointestinal lesions. Acetylsalicylic acid weaken the action of diuretics, antihypertensive drugs, as well as anti-gout agents that promote the excretion of uric acid; increases the risk of developing unwanted reactions from the gastrointestinal tract when used with other NSAIDs. Acetylsalicylic acid increases the concentration of digoxin, barbiturates and lithium salts in blood plasma. With simultaneous use with antacids containing magnesium and / or aluminum hydroxide, the absorption of acetylsalicylic acid slows down and deteriorates.

    Ascorbic acid improves the absorption of iron in the intestine.

    Special instructions:

    Acetylsalicylic acid reduces the excretion of uric acid from the body, which can cause an acute attack of gout in predisposed patients.

    With prolonged use of the drug should periodically make a general blood test and analysis of feces for latent blood, monitor the functional state of the liver.

    Because the acetylsalicylic acid slows down the coagulation of blood, the patient, if he is to undergo surgery, must warn the doctor in advance about taking the drug.

    During treatment, you should stop using alcohol (increased risk of gastrointestinal bleeding).
