    ATX: & nbsp

D.07.C.A.01   Hydrocortisone in combination with antibiotics

    Pharmacodynamics:

Combination drug for external use. Hydrocortisone - glucocorticosteroid, has anti-inflammatory, anti-allergic, antiexudative and antipruritic effects. Brakes phagocytosis. Reduces the formation of arachidonic acid from the phospholipids of cell membranes due to inhibition. activity of phospholipase A2.Oppresses the release of interleukins and other cytokines that have an anti-inflammatory effect; inhibits the release of histamine and the occurrence of local allergic reactions. Reduces the synthesis of proteins and the deposition of collagen in the skin, prevents the development of connective tissue, weakens proliferative processes.

Oxytetracycline - an antibiotic from the group of tetracyclines, has a bacteriostatic effect. It is active against Gram-positive and Gram-negative bacteria, Rickettsia spp., Treponema spp., Chlamydia spp., Mycoplasma spp.
Pharmacokinetics:With external use of the drug in therapeutic doses, transdermal absorption of active substances into the blood is very small. The use of occlusive dressings increases the transdermal absorption of hydrocortisone and oxytetracycline, which can lead to an increased risk of systemic side effects.
    Indications:

Diseases of the skin, amenable to therapy with glucocorticosteroids and complicated by a secondary bacterial infection caused by microorganisms sensitive to oxytetracycline: dermatitis (including allergic, contact, seborrhoeic, solar, atopic), eczema, multiforme, exudative erythema, superficial burns and frostbites, insect bites.

    Contraindications:

Hypersensitivity, skin tuberculosis, skin tumors, precancerous skin diseases, dermatomycosis, viral skin diseases (herpes, chicken pox), rosacea and perioral dermatitis, syphilitic skin lesions, postvaccinal skin reactions; pregnancy, lactation, children under 2 years.

    Dosing and Administration:

Outwardly. Apply a thin layer of 0.5-1 g 1-3 times a day on the affected areas or on a sterile gauze, which is covered with a skin, bandaged with a bandage. The duration of treatment is individual and depends on the degree of lesion and localization.

The course of treatment should not exceed 14 days. When applied to the skin of the face, the ointment is applied once a day, avoiding application to the periorbital area, and the course of treatment should not exceed 7-10 days.

Children are used in limited areas of the skin for a short course (for 5-7 days).

    Side effects:

Skin irritation, itching, rash; superinfection, allergic reactions. When applied to the skin of the face: telangiectasia, atrophy of subcutaneous tissue in the mouth. With long-term use and / or application on large surfaces - systemic side effects of glucocorticosteroids.

    Overdose:

Symptoms: with prolonged use and / or in high doses, it is possible to suppress adrenal function with the development of secondary adrenal insufficiency and symptoms of hypercorticism, including Itenko-Cushing syndrome.

Long-term treatment with oxytetracycline in elevated doses can lead to an increase in insensitive microflora.

Treatment: cancellation of the drug. Appropriate symptomatic therapy. If necessary - correction of electrolyte disturbances.

    Interaction:

Interactions of the drug with other drugs have not been identified.

    Special instructions:

During treatment with glucocorticosteroids, vaccination against smallpox should not be carried out and other immunizations should be planned (in connection with the immunosuppressive effect of glucocorticosteroids).

Avoid contact with eyes and mucous membranes. Children should avoid applying the drug on large areas of the skin and under occlusive dressings.

If it is necessary to apply the drug on the skin of the face, treatment should be short because of increased absorption and the possibility of side effects.
