    A.12.A.X   Calcium preparations in combination with other drugs

    Pharmacodynamics:

    Combined drug, the effect of which is due to its constituent components. Regulates the exchange of calcium and phosphate ions, reduces resorption and increases the density of bone tissue, replenishes the lack of calcium ions and vitamin D3 in the body, enhances the absorption of calcium ions in the intestine and the reabsorption of phosphates in the kidneys, promotes the mineralization of bones.

    Calcium carbonate - is involved in the formation of bone tissue, blood clotting, in the maintenance of stable cardiac activity, in the process of transmission of nerve impulses. The use of calcium and vitamin D3prevents the increase in the production of parathyroid hormone, which is a stimulator of increased bone resorption.
    Pharmacokinetics:

    Vitamin D3 absorbed in the small intestine.Calcium is absorbed in ionized form in the proximal part of the small intestine by means of an active transport mechanism.

    Indications:

    - Prevention and treatment of calcium and / or vitamin D deficiency3:

    - with insufficient intake of calcium and vitamin D in the body against a background of malnutrition

    - with increased body requirement for calcium and vitamin D3 during pregnancy and breastfeeding, as well as in children over 12 years of age during a period of intensive growth.

    - Prevention and as part of complex therapy of osteoporosis (menopausal, senile, "steroid", idiopathic, etc.)

    Contraindications:

    - Hypercalcemia (elevated concentration of calcium in the blood).

    - Hypercalciuria (increased calcium in the urine).

    - Nephrourolithiasis.

    - Hypervitamia of vitamin D.

    - Hypersensitivity to the components of the drug.

    - Severe kidney failure.

    - Active form of tuberculosis.

    - Sarcoidosis.

    - Osteoporosis due to immobilization.

    - Urolithiasis disease.

    - Atherosclerosis.

    - Children under 12 years.

    Carefully:Renal failure, benign granulomatosis, the intake of glycosides and thiazide diuretics, pregnancy, lactation.
    Pregnancy and lactation:

    The daily dose should not exceed 1500 mg of calcium and 600 ME vitamin D3. Hypercalcemia, which develops against the background of an overdose during pregnancy, can cause defects in the child's mental and physical development.

    Vitamin D and its metabolites can penetrate into breast milk, so you must take into account the intake of calcium and vitamin D from other sources in the mother and child. Overdose during pregnancy can lead to disruption of the child's mental and physical development.

    Dosing and Administration:

    Adults and children over 12 years: 2-3 capsules 2 times a day in the morning and in the evening, mainly during meals.

    Side effects:

    Allergic reactions, dysfunction of the gastrointestinal tract (constipation or diarrhea, flatulence, nausea, abdominal pain), hypercalcemia and hypercalciuria (elevated calcium in the blood or urine).

    Overdose:
    Symptoms of an overdose: anorexia, thirst, polyuria, decreased appetite, dizziness, fainting, weakness, nausea, vomiting, hypercalciuria, hypercalcification, hypercreatinemia. With long-term use - calcification of blood vessels and tissues.Treatment: the introduction of a large amount of fluid in the body, the use of loop diuretics (eg Furosemide), glucocorticosteroids, calcitonin, bisphosphonates. In case of signs of overdose, consult a physician.
    Interaction:

    - Vitamin D activity3 can decrease with its simultaneous use with phenytoin or barbiturates.

    - With simultaneous treatment with cardiac glycosides, ECG and clinical monitoring are necessary, calcium preparations can potentiate the therapeutic and toxic effects of cardiac glycosides.

    - Preparations of calcium and vitamin D3 can increase the absorption of tetracyclines from the gastrointestinal tract. Therefore, the time interval between taking the drug tetracycline series and Calcium-D3-MIC must be at least 3 hours.

    - To prevent the absorption of bisphosphonate or sodium fluoride drugs, it is recommended to take Calcium-D3-MIC not earlier than 2 hours after their admission.

    - Glucocorticosteroids reduce the absorption of calcium, so treatment with glucocorticosteroids may require an increase in the dose of Calcium-D-MIC.

    - Simultaneous treatment with Colustiramine or laxatives based on mineral or vegetable oil can reduce the absorption of vitamin D3.

    - With the simultaneous use of thiazide diuretics, the risk of hypercalcemia increases, because they increase the tubular reabsorption of calcium. Furosemide and other loop diuretics, on the contrary, increase the excretion of calcium by the kidneys.

    - In patients who simultaneously take cardiac glycosides and / or diuretics, it is necessary to monitor the concentration of calcium and creatinine in the serum.

    Special instructions:

    - During the treatment period, it is necessary to continuously monitor the excretion of calcium ions in the urine and the concentration of calcium and creatinine ions in the plasma (in the case of calcaruria, exceeding 7.5 mmol / day (300 mg / day), it is necessary to reduce the dose or stop taking).

    - To avoid an overdose, additional intake of vitamin D3 from other sources.

    - The intake of food containing oxalates (sorrel, spinach) and fitin (cereals) reduces the absorption of calcium, so you should not take Calcium-D3-MIC for two hours after taking sorrel, spinach, cereals.
