    АТХ:

    A.06.A.D.15   Macrogol

    Pharmacodynamics:

Osmotic laxative. It is a substance polyethylene glycol with a large molecular weight (3350 or 4000), which due to hydrogen bonds keeps water molecules in the lumen of the intestine. Increases the osmotic pressure and volume of the intestinal fluid, enhancing peristalsis without exerting an irritating effect. Increases the volume of chyme, prevents the loss of electrolytes with feces. It restores the reflex of evacuation, does not change the pH of the chyme. The laxative effect occurs 24-48 hours after oral administration.

Pharmacokinetics:It is not absorbed from the digestive tract and is not metabolized.Elimination by the kidneys (slightly, less than 0.1%).
Indications:Symptomatic treatment of constipation in children and adults; preparation for diagnostic studies (endoscopic, roentgenologic) and surgical interventions on the large intestine.

    XI.K55-K63.K59.0   Constipation

XXI.Z40-Z54.Z51.4   Preparatory procedures for subsequent treatment, not elsewhere classified

Contraindications:Hypersensitivity, severe diseases of the small and large intestine (obstruction, perforation of the intestinal wall, pronounced distention of the colon, severe inflammatory bowel disease); abdominal pain of unknown origin; dehydration; chronic heart failure in the stage of decompensation; erosive and ulcerative lesions of the mucous membrane of the colon (ulcerative colitis, Crohn's disease); disturbance of kidney function, children's age (for some medicinal forms).
Carefully:Violation of the swallowing reflex, reflux-esophagitis, depression of consciousness, pregnancy, children's age, advanced age.
Pregnancy and lactation:In experimental studies on animals, the teratogenic effect of macrogol was not revealed.Adequate and strictly controlled studies in pregnant women were not conducted. Use during pregnancy is possible on the recommendation of a doctor if the intended benefit for the mother exceeds the risk for the child.
The systemic effect of macrogol on the organism of nursing women is insignificant, therefore it can be applied during the period of breastfeeding.
Action category for the fetus by FDA - C.
Dosing and Administration:Is taken orally, previously dissolved in water.
    Adults:
As a laxative: 10-20 g / day, preferably in the morning.
Preparation for diagnostic examination, surgical interventions: on an empty stomach for 18-20 h prior to the examination or operation. Take in the form of a solution in a volume of 3-4 liters (200 ml every 20 minutes). During the reception and after it is necessary to use only liquid food.
    Children:
    As a laxative: from 6 months to 1 year - 4 g / day; from 1 year to 4 years - 4-8 g / day; from 4 to 8 years - 8-16 g / day.
The recommended course of treatment for children and adults is 3 months. Medicamental treatment of constipation is advisable to maintain a diet rich in plant fiber, and adequate physical activity. If the symptoms of constipation persist for more than 3 months, a re-expanded diagnostic examination should be performed.
Side effects:From the digestive system: sensation of anterior abdominal wall stretching, abdominal pain, nausea, vomiting, flatulence, diarrhea, irritation of the perianal area.
Allergic reactions: urticaria, itchy skin, rash, swelling of the skin and / or subcutaneous tissue, edema of the face, swelling of the Quincke; in some cases - anaphylactic shock.
Overdose:Exceeding the dose can lead to a violation of the water-electrolyte balance.
Symptoms: Diarrhea, which stops when the macrogol dose is reduced or canceled. Treatment: temporary withdrawal of the drug or dose reduction.
Interaction:With simultaneous ingestion of other drugs, it is possible to reduce their absorption. Considering this, macrogol should be taken 2 hours after taking other drugs.
Special instructions:Before the start of therapy, organic gastrointestinal diseases should be excluded. It is not recommended to use macrogol for a long time. Older patients with various concomitant diseases in preparation for diagnostic studies macrogol recommend to use under medical supervision.
In the case of diarrhea with extreme caution should be treated in patients, predisposed to violations of water-electrolyte balance (for example, patients with impaired liver or kidney function or patients receiving diuretics); in such cases it is necessary to control the content of electrolytes.
There are reports of aspiration cases when large volumes of macrogol are introduced with electrolytes through the nasogastric tube. Children with neurological disorders with swallowing are at risk of aspiration.
