    Pharmacodynamics:

    The drug is a loratadine metabolite. Competitively reversibly blocks histamine H1receptors, and therefore prevents effects that are mediated by histamine.

    Suppresses the cascade of cytotoxic reactions: the release of proinflammatory cytokines, including interleukin-4, interleukin-6, interleukin-8, interleukin-13, superoxide anion production by activated polymorphonuclear neutrophils adhesion and chemotaxis eosinophils, adhesion molecule expression, including P-selectin, IgE-dependent release of histamine, prostaglandin D2 and leukotriene C4.

    Pharmacokinetics:

    The maximum concentration of the drug in the plasma is achieved in 2-6 hours (an average of 3 hours). Practically it does not penetrate the blood-brain barrier, bind to plasma proteins at 82-87%, half-life - 27 hours, in patients older than 65 years - 33.7 hours. Elimination through the gastrointestinal tract and the kidneys - 87%.

    Indications:

    Chronic idiopathic urticaria, seasonal allergic rhinitis.

    Contraindications:

    Pregnancy, breast-feeding, hypersensitivity, age under 1 year, Children's age (up to 12 years) for tableted forms.

    For syrup (in addition, due to the presence of sucrose and sorbitol in the composition): hereditary intolerance to fructose, impaired absorption of glucose / galactose or insufficiency of sucrose / isomaltose.

    Carefully:

    Chronic diseases of the kidney and / or liver.

    Pregnancy and lactation:

    Category FDA - B. Adequate and well-controlled studies on humans have not been conducted. Penetrates into breast milk. The use is not recommended due to increased excitability in infants. Can inhibit lactation due to the presence of anticholinergic activity.

    Desloratadine is contraindicated in pregnancy and lactation (breastfeeding).

    Dosing and Administration:

    Orally.

    Adults-5 mg once a day.

    Children from 1 year to 5 years - 1.25 mg once a day.

    Children from 6 to 11 years - 2.5 mg once a day.

    Side effects:

    Headache, fatigue, dry mouth, dizziness, weakness, myalgia, dyspepsia (discomfort or pain in the stomach, heartburn, belching), nausea, dysmenorrhea.

    Overdose:

    Anticholinergic effects: clumsiness or instability, dryness in the oral cavity, nose or throat, flush of blood to the face, shortness of breath.

    CNS depression (drowsiness) or CNS stimulation (hallucinations, convulsions, restless sleep).

    Hypotension.

    Treatment is symptomatic. Do not use respiratory analeptics due to the threat of seizures!

    Interaction:

    Means depressing the central nervous system, alcohol - potentiating oppression.

    Anticholinergic drugs - increased anticholinergic effects, paralytic obstruction of the intestine.

    Ketoconazole and cimetidine, azithromycin, miconazole, metronidazole, intraconazole, erythromycin, fluconazole, fluoxetine - an increase in loratadine in plasma.

    Photosensitizing agents - enhancing their effect.

    Allergens for skin tests - it is not allowed to combine the drug with the application of skin tests.

    Monoamine oxidase inhibitors - the prolongation of the inhibitory effect on the central nervous system,as well as the anticholinergic effects of the drug.

    Special instructions:

    Sedation only at high doses does not cause ECG changes.

    With caution appoint desloratadine with severe renal failure.

    Impact on the ability to drive vehicles and manage mechanisms

    There was no adverse effect on driving or complicated technical devices.
