    Pharmacodynamics:M-cholinomimetic, has a miotic and antiglaucomal effect.
    Causes the contraction of the circular (miosis) and ciliary muscles (spasm of accommodation), increases the angle of the anterior chamber of the eye (the iris root is drawn back), increases the permeability of the trabecular area (trabecula stretches, and the blocked sections of the Schlemm's canal open), improves outflow of watery moisture from the eye and eventually reduces intraocular pressure.
    With primary open-angle glaucoma, instillation of 1% solution causes a decrease in intraocular pressure by 25-26%.The onset of the effect - after 30-40 minutes, reaches a maximum after 1.5-2 hours and lasts for 4-14 hours.
    Systems with a long-term release of pilocarpine provide control of intraocular pressure for 1 day, while the induced myopia developing during the first hours rapidly decreases and usually does not exceed 0.5 diopters.


    Pharmacokinetics:Well penetrates through the cornea. In the conjunctival sac is almost not absorbed. Pilocarpine well absorbed through the conjunctiva.
    The time required to reach the maximum concentration in the intraocular fluid is 30 minutes. It is retained in the tissues of the eye, which prolongs its half-life from the tissues of the eye, which is 1.5-2.5 hours.
    It is excreted unchanged with intraocular fluid.
    The system with long-term release of pilocarpine (eye film), moistened with tear fluid, passes into an elastic (soft) state and is retained in the lower conjunctival arch until it dissolves completely. The release of the active substance begins immediately after contact of the film with the conjunctiva.

    Indications:acute attack of angle-closure glaucoma; secondary glaucoma; (vascular,post-traumatic (burns)), primary open-angle glaucoma (in combination with beta-blockers or other drugs that reduce intraocular pressure); the need to narrow the pupil after instillation midriatikov.

    Contraindications:Individual sensitivity to the components of the drug, irit, cyclite, iridocyclitis, keratitis, the condition after ophthalmic operations and other eye diseases, in which the narrowing of the pupil is undesirable. Children under 18 years. Pregnancy, the period of breastfeeding.

    Carefully:In patients with retinal detachment in history and in young patients with high degree of myopia.

    Dosing and Administration:Bury in the conjunctival sac 1-2 drops 1-3 times a day. The number of instillations can vary depending on the indications and individual sensitivity of the patient.
    For the treatment of an acute attack of angle-closure glaucoma, during the first hour pilocarpine solution is instilled 1 drop every 15 minutes, for 2-3 hours every 30 minutes, for 4-6 hours every 60 minutes and further 3-6 times a day, until arresting an attack.
    If necessary, the narrowing of the pupil after instillation of mydriatic is instilled into the conjunctival sac 1-2 drops 2-6 times a day.


    Side effects:From the side of the organ of vision: local reactions - pain in the eye area; myopia, decreased vision, especially in the dark (due to the development of persistent miosis and spasm of accommodation); lacrimation, superficial keratitis. With prolonged use, it is possible to develop follicular conjunctivitis, contact eyelid dermatitis. Rarely - nuclear cataract, corneal endothelial edema, keratopathy, retinal detachment.
    System: headache (in temporal or periorbital areas), rhinorrhea, bronchospasm, hypersection, vomiting, diarrhea, increase or decrease in blood pressure, allergic reactions.

    Overdose:With an overdose of the drug develops persistent miosis, pain in the eye, visual impairment, headache, possibly the development of bronchospasm. When these symptoms appear, the drug is canceled, instillation of antagonist drugs - tropicamide, atropine or other derivatives thereof is performed.

    Interaction:Pilocarpine antagonists are atropine and other M-anticholinergic agents. With simultaneous use with adreno-mimetics - the antagonism of action (the diameter of the pupil).
    Timolol and phenylephrine increase the decrease in intraocular pressure (reduce the production of intraocular fluid).
    Possible use in combination with sympathomimetics, beta-blockers, inhibitors of carbonic anhydrase.
    M-cholinomimetic activity is reduced by tricyclic antidepressants, phenothiazine derivatives, chlorprotixen, clozapine; is increased by cholinesterase inhibitors. Possible development of bradycardia and lowering blood pressure during general anesthesia with halothane (in patients using pilocarpine in the eye drops).

    Special instructions:Treatment should be performed with regular monitoring of intraocular pressure.
    The use of pilocarpine is not recommended when wearing soft contact lenses.
    To reduce absorption to the systemic blood stream, it is recommended to squeeze the lacrimal canal for 1-2 minutes after instillation, pressing the finger at the inner corner of the eye in the area of ​​the projection of the lacrimal sac.
    After opening, use within 14 days.


    Effect on the ability to drive transp. cf. and fur:In the presence of an initial cataract, the miotic effect can cause a temporary visual impairment, therefore, during the treatment period, care must be taken when driving vehicles and practicing other potentially hazardous activities,requiring increased concentration of attention and speed of psychomotor reactions.

    Form release / dosage:Eye drops 1%.

    Packaging:1 ml, 1.5 ml, 2 ml in a dropper with a valve or 5 ml, 10 ml in a tube with a screw neck, made by the technology of "blowing - filling - sealing" of high-density polyethylene or low-density polyethylene pressure, or from polyethylene for medical purposes, or from polyethylene or polypropylene for infusion solutions and eye drops. Tube-dropper with a screw neck is capped with screw caps made of polymer materials.
