Pilocarpine-Ferein - instructions for use, dose, side effects, contraindications

Pharmacodynamics:Pilocarpine excites peripheral M-holinoretseptory and causes increased secretion of digestive and bronchial glands, a sharp increase in sweating, narrowing of the pupil, an increase in the tone of smooth muscles of the bronchi, intestines, bile and bladder, uterus.
The narrowing of the pupil is accompanied by the opening of the angle of the anterior chamber (the iris root is drawn back), the increase in the tone of the iris and the ciliary body, as a result of which the trabecula is stretched and the blocked sections of the helmet canal are opened.Due to the opening of the anterior chamber angle and the increase of the permeability of the trabecular diaphragm, intraocular pressure decreases. The action of pilocarpine on the level of intraocular pressure lasts for 4-6 hours.

Pharmacokinetics:Pilocarpine well penetrates through the cornea. With topical application, its concentration in the aqueous humor of the eye reaches a maximum after 30 minutes after instillation. It binds many tissues of the eye, which increases its half-life from the eye, which is 1.5-2.5 hours.
Pilocarpine is not metabolized in the eye tissues and is excreted with intraocular fluid.
Pilocarpine is converted into an inactive form by hydrolysis in blood serum and liver. The half-life of the plasma is about 30 minutes.

Indications:acute attack of angle-closure glaucoma;
- chronic angle-closure glaucoma;
- primary open-angle glaucoma (as a monotherapy and in combination with beta-adrenoblockers or other drugs that reduce intraocular pressure);
- if necessary, narrowing of the pupil after instillation of mydriatic (except for people with high degree myopia).

Contraindications:Increased individual sensitivity to pilocarpine; iritis, cyclilit, iridocicle, keratitis and other eye diseases, in which the narrowing of the pupil is not desirable.
Pilocarpine should be used with caution in patients with a history of retinal detachment and in young patients with high degree of myopia.

Pregnancy and lactation:It is possible to use Pilocarpine for the treatment of children, pregnant and lactating mothers, as directed by the attending physician, if the expected curative effect exceeds the risk of possible side effects.

Dosing and Administration:Bury in the conjunctival sac 1 drop 1-3 times a day. The number of instillations can vary depending on the indications and individual sensitivity of the patient.
To treat an acute attack of angle-closure glaucoma during the first hour, pilocarpine solution is instilled every 15 minutes, 2-3 hours every 30 minutes, 4-6 hours every 60 minutes and then 3-6 times a day until the seizure is stopped.

Side effects:Headache (in the temporal and periorbital regions), pain in the eye area, myopia; decreased vision, especially in the dark, due to the development of persistent miosis and spasm of accommodation; lacrimation, rhinorrhea, superficial keratitis; allergic reactions.
With prolonged use, it is possible to develop follicular conjunctivitis, contact eyelid dermatitis.

Overdose:Data on drug overdose are absent.

Interaction:Pilocarpine antagonists are atropine and other M-anticholinergic agents. With simultaneous use with adrenostimulators, antagonism of action (per pupil diameter) can be observed. Timolol and mezaton increase the decrease in intraocular pressure, reducing the production of intraocular fluid.
Possible use in combination with sympathomimetics, beta-blockers, inhibitors of carbonic anhydrase.
M - cholinostimulating activity of Pilocarpine-Ferein® is reduced by tricyclic antidepressants, phenothiazine derivatives, chlorprotixen, kappazin; is enhanced by anticholinesterase agents.
It is possible to develop bradycardia and lower blood pressure during general anesthesia with fluorotan (in patients using pilocarpine in the eye drops).

Special instructions:Due to the development of persistent miosis, as well as the myopic effect associated with a change in accommodation, it is possible to reduce visual acuity,so the use of the drug is not recommended for drivers of vehicles in the evening and at night.

Form release / dosage:Eye drops 10 mg / ml.

Packaging:To 5 or 10 ml in bottles, hermetically sealed with stoppers rubber or from a rubber compound with the subsequent obakachkoj caps aluminum or in plastic bottles with lids-droppers.
