    Pharmacodynamics:Pilocarpine excites peripheral M-cholinergic receptors and causes a narrowing of the pupil. The narrowing of the pupil is accompanied by the opening of the angle of the anterior chamber (the iris root is drawn back), the increase in the tone of the iris and the ciliary body, as a result of which the trabecula is stretched and the blocked sections of the helmet canal are opened. Due to the opening of the anterior chamber angle and the increase of the permeability of the trabecular diaphragm, intraocular pressure decreases.The action of pilocarpine on the level of intraocular pressure lasts for several hours (4-6 hours).

    Pharmacokinetics:Pilocarpine well penetrates through the cornea. With topical application, its concentration in the aqueous humor of the eye reaches a maximum after 30 minutes after instillation. It binds many tissues of the eye, which increases its half-life from the eye, which is 1.5-2.5 hours.
    Pilocarpine is not metabolized in the eye tissues and is excreted with intraocular fluid.
    Pilocarpine is converted into an inactive form by hydrolysis in blood serum and liver. The half-life of the plasma is about 30 minutes.

    Indications:acute attack of angle-closure glaucoma;
    - chronic angle-closure glaucoma;
    - primary open-angle glaucoma (as ionotherapy and in combination with beta-blockers or other drugs that reduce intraocular pressure);
    - if necessary, narrowing of the pupil after instillation of mydriatic (except for people with high degree myopia).

    Contraindications:Increased individual sensitivity to pilocarpine; iritis, cyclit, iridocyclitis, keratitis and other eye diseases, in which the narrowing of the pupil is not desirable.
    Pregnancy, lactation, children under 18 years.

    Carefully:In patients with retinal detachment in history and in young patients with high degree of myopia; severe cardiovascular failure.

    Pregnancy and lactation:

    Dosing and Administration:Bury in the conjunctival sac 1 drop 1-3 times a day. The number of instillations can vary depending on the indications and individual sensitivity of the patient.
    For the treatment of an acute attack of angle-closure glaucoma, during the first hour pilocarpine solution is instilled every 15 minutes, 2-3 hours every 30 minutes, 4-6 hours every 60 minutes and further 3-6 times a day, until the seizure occurs.

    Side effects:Headache (in the temporal and periorbital regions), pain in the eye area, myopia; decreased vision, especially in the dark, due to the development of persistent miosis and spasm of accommodation; lacrimation, rhinorrhea, superficial keratitis; allergic reactions.
    With prolonged use, it is possible to develop follicular conjunctivitis, contact eyelid dermatitis.

    Overdose:When an overdose of the drug develops persistent miosis, pain in the eye, visual impairment, headache.
    When these symptoms appear, the drug is canceled, instillation of antagonist drugs - tropicamide, atropine or other derivatives thereof is performed.

    Interaction:Pilocarpine hydrochloride antagonists are atropine and other M-anticholinergic agents. With simultaneous use with adrenostimulators, antagonism of action (per pupil diameter) can be observed. Timolol and phenylephrine increase the decrease in intraocular pressure, reducing the production of intraocular fluid.
    It is possible to use pilocarpine in combination with sympathomimetics, beta-adrenoblockers, inhibitors of carbonic anhydrase.
    M-holinostimulating activity of pilocarpine is reduced by tricyclic antidepressants, phenothiazine derivatives, chlorprotixen, clozapine; is enhanced by anticholinesterase agents.
    It is possible to develop bradycardia and decrease blood pressure during general anesthesia with the use of fluorotane (in patients using pilocarpine in the eye drops).

    Special instructions:Treatment should be performed with regular monitoring of intraocular pressure. To reduce absorption, it is recommended after instillation to pinch the eye canal for 1-2 minutes, pressing the finger at the inner corner of the eye.

    Effect on the ability to drive transp. cf. and fur:In the presence of an initial cataract, the miotic effect can cause transient visual impairment, therefore, during the treatment period, care must be taken when driving vehicles and engaging in other potentially dangerous activities requiring increased attention and speed of psychomotor reactions.

    Form release / dosage:1% solution (eye drops) of 5 ml in glass bottles with a dropper cap.

    Packaging:1 bottle and lid-dropper along with instructions for use are placed in a cardboard pack.

    Storage conditions:List A. In the dark place at a temperature of no higher than 20 ° C. Keep out of the reach of children.

    Shelf life:2 years. After opening the bottle, use no more than 30 days.
