АТХ:R.05 Drugs used for cough and colds
Pharmacodynamics:Dextromethorphan blocks glutamatergic NMDA receptors in various structures of the brain, including in the cough center (noncompetitive blockade of the ion channel) of the brainstem, the agonist σ1- and σ2receptors of the brain (a hallucinogenic effect), as a result, suppresses the excitability of the cough center in the brain, inhibiting the cough reflex.
Terpinhydrate stimulates the secretory function of the epithelial glands of the respiratory tract, increases the amount of secretion and promotes the dilution of the secreted secret, provides protection of the inflamed respiratory epithelium from direct contact with the external environment, contributing to the acceleration of the healing processes.
Levomenthol displays an antispasmodic effect, which helps alleviate the symptoms of pharyngitis, rhinitis, bronchitis and laryngitis.
Pharmacokinetics:Dextromethorphan is completely absorbed in the gastrointestinal tract, metabolized in the liver, eliminated with urine in unchanged form and in the form of metabolites (45%). Maximum concentration in blood plasma - In 2 hours. The onset of action is 10-30 minutes after admission, duration - for 5-6 hours in adults and up to 6-9 hours for children.
Terpinhydrate is rapidly and completely absorbed from the gastrointestinal tract, circulates in the blood in an unchanged form, is eliminated through the secretion of the epithelium of the respiratory tract cells, as well as with urine and sweat, the other part is oxidized and excreted in the urine in the form of phenols. Levomenthol Eliminated with bile and urine in the form of glucuronide.

Contraindications:Bronchial asthma, hypersensitivity, pregnancy and breastfeeding, under 2 years of age, sugar / isomaltase deficiency, fructose intolerance, glucosogalactose malabsorption (due to the presence of sucrose).
Carefully:Violation of the function of the liver.
Pregnancy and lactation:Category Food and Drug Administration (US Food and Drug Administration) not determined. The use of the drug is contraindicated in pregnancy. For the duration of treatment, breastfeeding should be discontinued.
Dosing and Administration:1 ml of syrup contains dextromethorphan - 2 mg, levomentol - 0.75 mg and terpinhydrate - 2 mg. Take orally.
Adults: 1 teaspoonful (5 ml) 3-4 times a day.
Children from 4 to 6 years - 1/4 teaspoon 3-4 times a day; from 7 to 12 years - 1/2 teaspoon 3-4 times a day.

Side effects:Hives, itching, nausea, drowsiness, dizziness.
Overdose:Symptoms: agitation, dizziness, respiratory depression, decrease blood pressure, tachycardia, dyspeptic disorders.
Treatment: artificial ventilation, symptomatic treatment. Specific antidote - naloxone, can be used for an overdose of 100 or more.
Interaction:The drug enhances the effect of narcotic antitussive drugs, the effect of which is to suppress the central nervous system.
The drug can interact with monoamine oxidase inhibitors.
Special instructions:When using the drug there is no development of addiction.
Terpin hydrate, which is part of the drug, gives the urine and sweat a specific odor, which has no clinical significance.
