    Composition:

    1 tablet contains:

    Active substance - furosemide 40 mg.

    Auxiliary substances: lactose (sugar milk), potato starch, magnesium stearate - a sufficient amount to obtain a tablet weighing 150 mg.

    Description:

    Tablets are white with a yellowish hue of color, flat-cylindrical with a bevel.

    Pharmacodynamics:

    High-speed diuretic with pronounced diuretic effect. It disrupts the reabsorption of sodium, chlorine ions in the thick segment of the ascending part of the Henle loop. Furosemide has a pronounced diuretic, natriuretic, chlororetic effect. In addition, it increases the excretion of potassium, calcium, and magnesium ions.

    The action of furosemide after ingestion comes in 30-60 minutes. The maximum effect of the drug - after 1-2 hours duration of the effect is usually 2-3 hours (depending on the function of the kidneys to 8 hours).

    Indications:

    edematous syndrome of various genesis, including chronic heart failure IIB-III stages, cirrhosis of the liver (syndrome of portal hypertension); edematous syndrome of renal genesis (with nephrotic syndrome in the foreground is the treatment of the underlying disease); acute left ventricular failure (cardiac asthma); edema caused by burns; arterial hypertension.

    Use strictly according to the doctor's prescription to avoid complications.

    Contraindications:

    Acute glomerulonephritis, acute renal failure with anuria, disturbed water-salt metabolism and KSHR (hypokalemia, alkalosis, hypovolemia, hyponatremia), precomatric states, hepatic coma, diabetic coma.

    Dosing and Administration:

    The dosage regimen is set individually, depending on the indications, the clinical situation, the age of the patient. In the process, the dosage regimen is adjusted depending on the magnitude of the diuretic response and the dynamics of the patient's condition.

    The drug is administered by mouth in the morning, before meals. The initial dose for adults is 40 mg (1 table). With insufficient action, the dose is gradually increased to 80-160 mg per day (2-4 doses at intervals of 6 hours).After reducing edema, the drug is prescribed in smaller doses with a break of 1-2 days.

    Side effects:

    With long-term use, as well as with the appointment in high doses, it is possible to develop hypovolemia, dehydration, hypokalemia, hyponatremia, hypochloremia, hypokalemic metabolic alkalosis and as a result of these disorders can be observed - arterial hypotension, dizziness, dry mouth, thirst, cardiac arrhythmias, muscle weakness , cramps calf muscles. Also possible nausea, vomiting, diarrhea, dermatitis, hyperuricemia (with possible exacerbation of gout), hyperglycemia. When using high doses of furosemide in patients with severe violations of the excretory function of the kidney in certain cases are possible: deafness, visual impairment, paresthesia.

    Due to the increased loss of sodium (especially in conditions of insufficient salt intake), apathy, loss of appetite, a sense of weakness, lethargy, drowsiness, confusion may occur.

    Due to the loss of electrolytes and liquid, the already existing metabolic alkalosis may be aggravated.

    Allergic reactions are possible: skin rashes, itching, in some cases anaphylactic shock is possible.

    Interaction:

    It is inappropriate to combine furosemide with cephalosporins, gentamicin and other drugs that have a nephro- and ototoxic effect.

    Furosemide increases the concentration of levomycetin, aminoglycosides (streptomycin, gentamicin, neomycin and others), dephaloporins (cephaloridine, cephalexilate, etc.) in the blood. Therefore, when combined use of these drugs is likely to increase the therapeutic and undesirable actions of antibiotics, which will require a reduction in its dose. Long-term therapy with furosemide in large doses causes an elongation of the half-life of digitalis preparationsdigoxin, digitoxin and others) and strengthening their action.

    Under the influence of non-steroidal anti-inflammatory drugs, the diuretic effect of furosemide decreases.

    With the simultaneous use of furosemide and antihypertensive drugs (including ACE inhibitors), the hypotensive effect is potentiated.

    Special instructions:

    Patients with hypersensitivity to sulfonamides (sulfonylureas) can give reactions of cross-sensitivity to furosemide. During the treatment with furosemide, it is necessary to monitor blood pressure, the level of electrolytes and glucose in the blood serum,function of the liver and kidneys.

    Treatment with furosemide should be conducted against a background of a diet rich in potassium.
