B.05.B.B   Solutions affecting the water-electrolyte balance

    B.05.B.B.01   Electrolytes

    Pharmacodynamics:

Rehydrating agent, has detoxification effect, increases the content of sodium, potassium, calcium and chloride ions in the blood. Ringer's solution compensates for loss of extracellular fluid and basic electrolytes (sodium, potassium, calcium) and chloride.

    Pharmacokinetics:

Metabolic transformations in the body are not affected. Quickly excreted by the kidneys, within 4 hours 80% is output, and after 12-24 hours it is output completely. Information about the pharmacokinetics refers to the ions that make up the drug.

Indications:Hypovolemic shock, thermal burns of severe 3 and 4 degrees; hypovolemia; intestinal fistula; acute intestinal infections; therapeutic plasmapheresis; correction of water-salt balance in acute diffuse peritonitis and intestinal obstruction.
    Contraindications:

Hyperchloremia, hypercalcemia, hypernatremia, acidosis, chronic heart failure III-IV functional class for NYHA, pulmonary edema, cerebral edema, severe chronic renal insufficiency , concomitant therapy with glucocorticosteroids.

Carefully:Chronic heart failure I-II functional class for NYHA, arterial hypertension, chronic renal insufficiency easy and medium degree, impaired liver function.
Pregnancy and lactation:During pregnancy, apply in cases where the expected benefit to the mother exceeds the possible risk to the fetus. Caution should be used during breastfeeding due to the lack of appropriate clinical data.
    Dosing and Administration:

Enter intravenously drip, with a speed of 60-80 cap / min or jet. Daily Adult Dose - 5-20 ml / kg, if necessary, can be increased to 30-50 ml / kg. Daily dose for children -5-10 ml / kg, the injection rate of 30-60 cap / min, with shock dehydration, initially injected 20-30 ml / kg. The course of treatment is 3-5 days. For mild and moderate severity of acute intestinal infections, the drug is used only if oral rehydration is not possible.

When the therapeutic plasmapheresis solution was administered in a volume twice the volume of a remote plasma (1.2-2.4 L), in the case of severe hypovolemia - in combination with the colloidal solutions. The maximum volume of the injected solution is 3 liters / day.
    Side effects:Hyperhydration, hyperkalemia, allergic reactions.
    Overdose:

In case of an overdose, there may be a disturbance in the balance of water and electrolytes (hypervolemia, hypernatremia, hyperkalemia, hypercalcemia, hyperchloremia) and acid-base balance. In case of an overdose, in most cases, it's enough to interrupt introduction.

    Interaction:

When combined with other drugs, it is necessary to visually check compatibility.

It is possible to increase sodium retention in the body while taking the following medicines at the same time: non-steroidal anti-inflammatory drugs, androgens, anabolic hormones, estrogens, corticotropin, mineralocorticoids, vasodilators or ganglion blockers. When taking with potassium-sparing diuretics, angiotensin-converting enzyme (ACE) inhibitors and potassium preparations, the risk of hyperkalemia increases. In combination with cardiac glycosides, the likelihood of their toxic effects increases.
    Special instructions:

In the case of rapid introduction of a large volume, it is necessary to monitor the acid-base state and electrolyte content in blood. Change in blood pH (acidification) leads to redistribution TO+(lowering the pH leads to increase in the content of K+ in the blood serum). Freezing the drug, provided that the tightness of the container is maintained, is not a contraindication to the use of the drug. If the container contents become cloudy, do not use it.
