    B.03.A.A.05   Iron chloride

    Pharmacodynamics:

    Hemofer is an anti-anemic drug containing ferrous iron as a simple ferric chloride salt and intended for the treatment of iron deficiency conditions. Iron is necessary for normal functioning of the body, it is part of hemoglobin, myoglobin, cytochrome C, stimulates erythropoiesis; reversibly binds oxygen and participates in oxidation-reduction reactions.

    Iron chloride replenishes the existing iron deficiency in the body, and also counteracts the appearance of its deficiency in the period of increased need and in case of insufficient intake of iron.

    Pharmacokinetics:

    Iron ions are absorbed in the upper parts of the small intestine, mainly in the duodenum. The absorbed iron is retained in the ferritin-related form, mainly in the liver.In the bone marrow it is included in the hemoglobin.

    Absorption of iron depends on the valence, the degree of acidity of the gastric contents and iron deficiency in the body. Preparations of ferrous iron do not require introductory reduction and are easily absorbed.

    With a significant deficit, the degree of absorption of iron is about 20%. With decreasing deficit, iron absorption decreases.

    Indications:
    • Treatment of clinically pronounced iron deficiency anemia;
    • Treatment of latent iron deficiency;
    • Treatment of chronic posthemorrhagic anemia;
    • Prevention of iron deficiency during pregnancy and lactation; in premature babies; newborns, from multiple pregnancies; in children whose mothers had iron deficiency anemia during pregnancy; in the period of intensive growth in children and adolescents; for vegetarians and the elderly.
    Contraindications:
    • Hypersensitivity to any of the components of the drug;
    • Excessive iron content in the body (including hemosiderosis);
    • Anemia not related to iron deficiency (including hemolytic, megaloblastic anemia);
    • Anemia caused by impaired iron utilization (incl.sideroachrestic anemia, lead anemia);
    • Aplastic anemia;
    • Hemochromatosis;
    • Inflammatory diseases of the gastrointestinal tract;
    • Deficiency of sugar / isomaltase, intolerance to fructose, glucose-galactose malabsorption.
    Carefully:

    Stomach ulcer and duodenal ulcer, enteritis, ulcerative colitis.

    Pregnancy and lactation:

    With established iron deficiency, the use of The drug Hemofer during pregnancy and lactation is justified and safe.

    Dosing and Administration:

    The dosage regimen is set individually. The drug is taken orally, between - meals, with water or juice.

    In 1 ml contains 44 mg of elemental iron, in 1 drop - about 1.9 mg of elemental iron.

    For medicinal purposes:

    Premature babies:

    daily 1.5 to 3.0 mg / kg body weight for 3 to 5 months

    Children under 1 year old

    daily 15-30 mg (8-16 drops) per day

    Adults

    daily 2 ml (46 drops) 2 times a day

    When treatment of anemia the maximum dose for children is 3 mg / kg / day, for adults - 180-200 mg / day.

    Preventive doses are 30-50% of medical.

    The duration of therapy is at least 2 months. With clinically significant iron deficiency, the hemoglobin level normalizes only after 2-3 months after the start of treatment. Then, to restore iron stores in the body, Hemofer is prescribed in preventive doses for several months.

    Side effects:Pyrexia, headache, flushing of the skin, allergic reactions (itching, rash), nausea, vomiting, constipation or diarrhea, abdominal pain.
    Overdose:

    Symptoms: epigastric pain, nausea, vomiting, diarrhea or constipation, lowering blood pressure, in more severe cases - lethargy, pallor, necrosis of the mucous membranes of the gastrointestinal tract, hypovolemic shock even coma.

    Symptoms of peripheral circulatory collapse appear within 30 minutes after administration; metabolic acidosis, convulsions, fever, leukocytosis, coma - within 12-24 hours; acute renal and hepatic necrosis - after 2-4 days.

    Treatment: drug cancellation, gastric lavage, prescription of laxatives, symptomatic therapy; in case of severe poisoning - deferoxamine in / in slowly: children - 15 mg / h, adults - 5 mg / kg / h (up to 80 mg / kg / day); with mild poisoning in / m: children - 1 g every 4-6 h, adults - 50 mg / kg (up to 4 g / day).

    Interaction:

    Incompatible with drugs containing the SH group.

    Antacid medicines, bismuth tricalcium dicitrate, calcium phosphate, chloramphenicol, colestramine, Colestipol, pancreatin, zinc salts, tocopherol reduce the absorption of the drug; ascorbic acid increases.

    The drug may reduce the effectiveness of fluoroquinolones, levodopa, levothyroxine, methyldopa, tetracyclines, penicillin, sulfasalazine, as well as zinc salts. Simultaneous use of iron salts and allopurinol should be avoided.

    Food, mainly milk and dairy products, as well as eggs, reduce the absorption of the drug.

    Special instructions:

    The drug Hemofer can cause a dark color of the enamel of the teeth, so it should be taken through a straw or rinse your mouth immediately after taking the drug. During the period of taking the drug, the stool may be darker due to the increased iron content in the intestine.

    When a liquid stool appears, you can reduce a single dose of the drug, but increase the frequency of its appointment.

    Control of laboratory indicators

    During the treatment period, it is recommended that the concentration of hemoglobin and iron in the blood be monitored every 4 weeks. After reaching the normal values of hematological parameters, the drug should be used for another 6-8 weeks.

    Effect on the ability to drive transp. cf. and fur:

    The influence of the drug Hemofer on the ability to drive and perform work that requires increased attention has not been studied.
