    Composition:

    1 capsule contains:

    active substance - Loperamide hydrochloride 2.0 mg;

    Excipients: lactose monohydrate - 104.5 mg, corn starch -30.0 mg, silicon dioxide colloid -0.7 mg, talc 1.4 mg, magnesium stearate 1.4 mg.

    Capsule shell composition: Hard gelatin capsule (1.3333% titanium dioxide, 0.9197% color quinoline yellow, 0.0044% dye sunset yellow, 100% gelatin, 38.0 mg).

    Description:

    Capsules number 4 of yellow color. The contents of capsules are white or white powder with a yellowish tinge.

    Pharmacodynamics:

    Loperamide, binding to opioid receptors of the intestinal wall (stimulation of choline and adrenergic neurons through guanine nucleotides), reduces tone and motility of smooth intestinal muscles (by inhibiting the release of acetylcholine and prostaglandins). Slows down the peristalsis and increases the transit time of intestinal contents. Increases the tone of the anal sphincter, promotes retention of fecal masses and reduced urge to defecate. Action develops quickly and lasts 4-6 hours.

    Pharmacokinetics:

    When taken orally, the absorption is 40%. The maximum concentration in the plasma is achieved in 2.5 hours after taking the capsules.The connection with plasma proteins (mainly with albumins) is 97%. Does not penetrate the blood-brain barrier. It is almost completely metabolized by the liver by conjugation. The half-life is 9-14 hours (an average of 9.8 hours). It is excreted mainly with bile, an insignificant part is excreted by the kidneys (in the form of conjugated metabolites).

    Indications:

    Symptomatic treatment of acute and chronic diarrhea of ​​various genesis (allergic, emotional, drug, radiation, with a change in diet and quality of food, in violation of metabolism and absorption, as an auxiliary for diarrhea of ​​infectious genesis). Regulation of stool in patients with ileostomy.

    Contraindications:

    Hypersensitivity, intestinal obstruction, acute ulcerative colitis, diverticulosis, diarrhea on the background of acute pseudomembranous enterocolitis, dysentery and other infections of the gastrointestinal tract. Pregnancy (1 trimester), lactation period, children under 2 years of age (Loperamide-Akrihin capsules are not prescribed for children under 6 years of age).

    Dosing and Administration:

    Inside, not liquid, wash with water.

    Adults with acute diarrhea initially assigned 2 capsules (4 mg) Loperamide-Akrihin, then 1 capsule (2 mg) after each act of defecation in the case of a loose stool. The highest daily dose is 8 capsules (16 mg).

    In chronic diarrhea, adults are prescribed 4 mg / day. The maximum daily dose is 16 mg.

    In acute diarrhea, children over 6 years of age are given an initial dose of 2 mg, then 2 mg after each act of defecation in the case of a loose stool. The maximum daily dose is 4 capsules (8 mg).

    For chronic diarrhea, children over 6 years of age are prescribed Loperamide-Acrychin in a daily dose of 2 mg. The maximum daily dose of 6 mg per 20 kg.

    After normalization of the stool or in the absence of stool for more than 12 hours, the drug should be stopped.

    Side effects:

    It is observed, as a rule, only with prolonged intake of the drug.

    Possible allergic reactions (skin rash), drowsiness, dizziness, hypovolemia, electrolyte disorders, dry mouth, intestinal colic, gastralgia, pain or discomfort in the abdomen, nausea, vomiting, flatulence.

    Rarely - urinary retention, extremely rare - intestinal obstruction.

    Overdose:

    Symptoms: signs of inhibition of the central nervous system (stupor,impaired coordination of movements, drowsiness, narrowing of pupils (miosis), increased tone of skeletal muscles, respiratory depression), intestinal obstruction.

    Treatment: As a specific antidote used naloxone. Given that the duration of action of Loperamide-Acrychin is greater than that of naloxone, it is possible to re-introduce the latter. Symptomatic treatment: gastric lavage, taking activated charcoal (in the first 3 hours after taking the drug), artificial ventilation.

    Interaction:

    If there is no effect after 2 days of Loperamide-Akrihin, it is necessary to clarify the diagnosis and exclude the infectious genesis of diarrhea.

    Children under the age of 6 are not recommended to be given capsules.

    If the treatment develops constipation or bloating, loperamide-acrychin should be discarded. In patients with impaired liver function, careful monitoring of signs of toxic damage to the central nervous system is necessary. During the treatment of diarrhea, it is necessary to replenish the loss of fluid and electrolytes.

    Effect on the ability to drive transp. cf. and fur:During treatment, care must be taken when driving vehicles and engaging in other potentially hazardous activities requiring increased concentration of attention and speed of psychomotor reactions.
