A.11.G.B   Ascorbic acid in combination with other drugs

Pharmacodynamics:

Ascorbic acid plays an important role in the regulation of oxidation-reduction processes, carbohydrate metabolism, blood clotting, tissue regeneration, and improves the body's resistance. Ascorbic acid (vitamin C) is not formed in the human body, but comes only with food. With a balanced diet, a person does not experience a deficiency in vitamin C.

Dextrose is involved in various metabolic processes in the body, enhances oxidation-reduction processes in the body, improves the antitoxic function of the liver.

Pharmacokinetics:

It is absorbed completely by the body, the kidneys are not excreted (the appearance in the urine is a pathological sign).

Indications:

- Prevention and treatment of hypovitaminosis C;

- providing increased body requirement in vitamin C during growth, pregnancy, breastfeeding;

- with increased physical and mental stress, fatigue, stressful conditions, in the period of recovery after long and severe diseases.

Contraindications:

- Hypersensitivity to the active substance and components of the drug;

- Children's age (up to 3 years);

- deficiency of glucose-6-phosphate dehydrogenase.

Carefully:It should appoint a high doses to patients with increased blood clotting, thrombophlebitis and a tendency to thrombosis, as well as in diabetes mellitus and conditions associated with increased blood sugar.

When used in large doses - hemochromatosis, sideroblastic anemia, thalassemia, hyperoxaluria, nephrourolythiasis.

Pregnancy and lactation:

Ascorbic acid penetrates the placental barrier. It should be borne in mind that the fetus can adapt to high doses of ascorbic acid, which is taken by a pregnant woman, and then a newborn can develop withdrawal syndrome. Therefore, in pregnancy, do not take ascorbic acid in elevated doses, except when the expected benefit exceeds the potential risk. Ascorbic acid excreted in breast milk.A mother's diet containing an adequate amount of ascorbic acid is sufficient to prevent her deficiency in an infant. It is recommended not to exceed the maximum daily need for ascorbic acid by the nursing mother, unless the expected benefit exceeds the potential risk.

Dosing and Administration:

The drug is taken orally after a meal. Doses are given in terms of ascorbic acid. With the preventive purpose of appoint:

- adults 50-100 mg per day;

- children from 6 to 14 years - 50 mg per day, children from 14 to 18 years - 75 mg per day;

- during pregnancy and lactation, 300 mg per day for 10-15 days, then 100 mg per day;

With a therapeutic purpose appoint:

- adults 50-100 mg 3-5 times a day;

- children 50-100 mg 2-3 times a day.

The maximum daily dose for adults is 1000 mg, for children - 500 mg.

The timing of treatment depends on the nature and course of the disease and is recommended by the doctor.
Side effects:Immune system disorders: allergic reactions;

Disorders from the digestive system: irritation of the mucous membrane of the gastrointestinal tract (gastrointestinal tract) (nausea, vomiting, diarrhea, spasm of the gastrointestinal tract); oppression of the insulin pancreas apparatus (hyperglycemia, glucosuria);

When used in high doses:

Impaired kidney and urinary system: hyperoxaluria and nephrocalcinosis (oxalate);

Change in laboratory indicators: thrombocytosis, hyperprothrombinemia, erythropenia, neutrophilic leukocytosis, hypokalemia.

Overdose:

Symptoms: with the use of more than 1 g, the appearance of headache, increased central nervous system (CNS) excitability, insomnia, nausea, vomiting, diarrhea, hyperacid gastritis, ulceration of the gastrointestinal mucosa, oppression of the insulin pancreas apparatus (hyperglycemia, glucosuria); Hyperoxalaturia, nephrolithiasis (from calcium oxalate), damage to the glomerular apparatus of the kidneys, moderate pollakiuria (when taking a dose of more than 600 mg / day).

Reducing the permeability of capillaries (possibly worsening tissue trophism). Increased blood pressure (AD), hypercoagulation, development of microangiopathies.

In case of taking a potentially toxic dose inside, it is necessary to induce vomiting, in rare cases - wash the stomach, prescribe Activated carbon, laxatives (for example, isotonic sodium sulfate solution).

Interaction:

Ascorbic acid enhances concentration in the blood of benzylpenicillin and tetracyclines; in a dose of 1 g / day. enhances bioavailability of ethinyl estradiol (including those included in oral contraceptives).

Improves absorption in the intestine of iron preparations (converts trivalent iron into bivalent); can increase the excretion of iron with simultaneous use with deferoxamine.

Reduces efficacy of heparin and indirect anticoagulants.

Acetylsalicylic acid (ASA), oral contraceptives, fresh juices and alkaline drink reduce absorption and absorption of the drug.

With the simultaneous use of the drug with ASA increases urinary excretion of ascorbic acid and decreases excretion of ASA. ASA reduces absorption of ascorbic acid by about 30%.

Increases the risk of developing crystalluria in the treatment of short-acting salicylates and sulfonamides, slows down excretion of acids by the kidneys, increases the elimination of drugs that have an alkaline reaction (including alkaloids), reduces concentration in the blood of oral contraceptives.

Raises the the total clearance of ethanol, which in turn reduces concentration of ascorbic acid in the body.

The drugs of quinoline series, calcium chloride, salicylates, glucocorticoid agents with long-term use exhaust stocks of ascorbic acid.

With simultaneous application ascorbic acid reduces the chronotropic action of isoprenaline.

With prolonged use or use in high doses, it can interfere with the interaction of disulfiram and ethanol.

In high doses enhances excretion of mexiletine by the kidneys.

Barbiturates and primidon increase the excretion of ascorbic acid in the urine. Reduces therapeutic effect of antipsychotic drugs (neuroleptics) - phenothiazine derivatives, tubular reabsorption of amphetamine and tricyclic antidepressants.

Special instructions:

In connection with the stimulating effect of ascorbic acid on the synthesis of corticosteroid hormones, it is necessary to monitor the function of the kidneys and blood pressure.

With prolonged use of large doses, it is possible to suppress the function of the insulin apparatus of the pancreas, so during the treatment it is necessary to regularly monitor the functional capacity of the pancreas.

In patients with elevated iron levels in the body, ascorbic acid should be used in minimal doses.

Assigning ascorbic acid to patients with rapidly proliferating and intensely metastatic tumors can aggravate the course of the process. Ascorbic acid as the reductant can distort the results of various laboratory tests (blood glucose, bilirubin, the activity of "liver" transaminases and lactate dehydrogenase).

Patients with diabetes should take into account that one tablet contains about 0.06 bread units (XE), in a daily dose (5 tablets) - about 0.3 XE.

Effect on the ability to drive transp. cf. and fur:

The drug does not affect the ability to drive vehicles, work with precise mechanisms and engage in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.
