    Pharmacodynamics:

    A means for restoring the alkaline state of blood and correcting metabolic acidosis; increases alkaline reserves of blood.Sodium bicarbonate dissociates into a hydrocarbonate anion and a sodium cation. The hydrocarbonate anion binds hydrogen ions to form a carbonic acid that decomposes into water and carbon dioxide, which is then released by breathing. Sodium (Na+) is the main cation of extracellular fluid and plays an important role in electrolyte metabolism; Sodium bicarbonate increases the excretion of sodium and chlorine ions, increases osmotic diuresis; shifting to the alkaline side the reaction of bronchial mucus, increases bronchial secretion, reduces the viscosity of phlegm and improves its expectoration. Leaves urine, prevents precipitation of uric acid in the urinary tract.

    Pharmacokinetics:

    Indications:It is used for the purpose of arresting decompensated metabolic acidosis in various diseases and conditions (the absolute indication is a decrease in blood pH below 7.2).

    Dosing and Administration:

    Depending on the degree of acidosis, 4% sodium bicarbonate solution is used undiluted or diluted 5 % solution of dextrose or 0.9% solution of sodium chloride.

    Adults; To correct metabolic acidosis, usually inject 100 ml of a 4% solution or 200 ml of a 2% solution.

    Children, depending on the age, body weight and severity of acidosis, administered from 4 to 10 ml / kg 4 % solution or 8-20 ml / kg of 2% solution under the control of acid-base state of blood. The maximum daily dose for children under 2 years is 16 ml / kg (8 mEq / kg) 4 % solution.

    Side effects:

    With daily long-term use in / in drip introductions of 150-200 ml can develop alkalosis: decreased appetite, belching, nausea, vomiting, anxiety, headache, abdominal pain, tetanic convulsions, increased blood pressure. In these cases, you should take a break in the use of the solution.

    Interaction:

    When simultaneous application decreases, the excretion of amphetamine with urine due to the increase in the pH of urine under the influence of sodium bicarbonate.With simultaneous use with methotrexate, excretion of methotrexate with urine increases and its toxic effect on the kidneys decreases due to an increase in the pH of urine under the influence of sodium bicarbonate. Enhances the effect of antihypertensive drugs. The drug is incompatible with solutions: ascorbic acid, cisplatin, codeine phosphate, dobutamine, dopamine, magnesium sulfate, morphine sulfate, norephedrine bitartrate, potassium salt of penicillin G, streptomycin sulfate, tetracycline. Due to the increase in the pH of urine under the influence of sodium bicarbonate, a delay in excretion of ephedrine from the body is observed and the risk of side effects (tremor, anxiety, sleep disturbances, tachycardia) increases.

    Special instructions:

    It is not recommended to systematically apply it because of the possibility of alkalinization of urine and increase the risk of formation of phosphate stones.

    The shift of the acid-base state to the alkaline side during short-term admission is not accompanied by clinical symptoms, but with chronic renal failure may significantly worsen the condition. Vomiting, often accompanying peptic ulcer disease (loss of SG ions), can increase the severity of alkalosis.

    In patients with concomitant diseases of the heart or kidneys, excessive ion intake Na+causes swelling and heart failure. When used to correct acidosis, it is necessary to monitor the acid-base state of the blood plasma.
