    Pharmacodynamics:

    A means for restoring the acid-base state of blood and correcting metabolic acidosis. During the dissociation of sodium Hydrocarbonate, a hydrocarbonate ion (HC03 ), which connects hydrogen ions to form carbonic acid, which then decomposes into water and exhaled carbon dioxide. As a result, the pH of the blood is shifted to the alkaline side, increasedthe buffer capacity of the blood. The drug also increases the excretion of sodium and chlorine ions from the body, increases osmotic diuresis, alkalinizes urine, preventing precipitation of uric acid salts in the urinary tract. Inside the cells, the hydrogen carbonate ion does not penetrate.

    Indications:

    Decompensated metabolic acidosis (including infections, intoxications, diabetes, postoperative period).

    Contraindications:

    Metabolic alkalosis, hypersensitivity, hypokalemia, hypernatremia.

    Carefully:During pregnancy, during lactation.
    Dosing and Administration:

    Intravenously (intravenously) is drip. Depending on the severity of acidosis apply undiluted or diluted in 0.9% solution of sodium chloride or 5% solution of dextrose.

    Adults with marked acidosis IV - 50-100 ml 42 mg / ml solution, with acute acidosis - 50-100 ml 84 mg / ml solution at a rate of 50-60 cap / min.Children under 5 years: 1 mEq / kg at a concentration of 42 mg / ml (0.5 mEq / ml), if necessary, repeat after 10 minutes at a dose of 0.5 mEq / kg; the rate of administration is not more than 1 ml / kg / min.

    HC03- (mEq) = 0.3 x weight (kg) x base deficit (mEq / l) =

    Children older than 5 years - 2 - 5 mEq / kg for 4 - 8 hours.

    Side effects:

    With daily long-term use in / in drops of 150-200 ml can develop alkalosis: decreased appetite, belching, nausea, vomiting, anxiety, headache, abdominal pain, tetanic convulsions, increased blood pressure. In these cases, you should take a break in the use of the solution.

    Overdose:

    When the dose is exceeded, development of decompensated alkalosis, tetanic convulsions is possible. With the development of alkalosis, stop the injection of the drug, with the risk of developing seizures, intravenously calcium gluconate.

    Interaction:

    In a solution of sodium bicarbonate it is impossible to dissolve substances of an acidic nature (ascorbic, nicotinic and other acids), alkaloids (atropine, apomorphine, caffeine, theobromine, papaverine and others), cardiac glycosides, salts of calcium, magnesium, heavy metals (iron, copper, zinc), as precipitation or hydrolysis of organic compounds occurs. Do not mix with phosphate-containing solutions. May enhance the effect of antihypertensive drugs.

    Special instructions:

    The shift of the acid-base state to the alkaline side during short-term administration is not accompanied by clinical symptoms, however, in chronic renal failure it can significantly worsen the condition. Vomiting, often accompanying an ulcer, (loss of chloride ions), can increase the severity of alkalosis.

    When used to correct acidosis, it is necessary to control the acid-base condition of the blood.

    It is not recommended regular use because of the possibility of alkalinization of urine and increase the risk of formation of phosphate stones.

    In patients with concomitant diseases of the heart or kidneys, excessive sodium intake causes swelling and heart failure.

    With chronic renal failure can significantly worsen the course of the disease.
