    R.06.A.X   Other antihistamines for systemic use

    Pharmacodynamics:Histamine H blocker1receptors, ethylenediamine derivative. Has antiallergic, antipruritic, antiexudative, and also weak sedation. It has weak m-holinoblocking properties. Penetrates through the blood-brain barrier in small amounts. The duration of the effect can reach 48 hours.
    Pharmacokinetics:After oral administration, it is rapidly absorbed from the digestive tract. Bioavailability is 40-60%. Virtually does not penetrate through blood-brain barrier. Metabolised in the liver by methylation. Causes the induction of microsomal liver enzymes. It is excreted by the kidneys.
    Indications:Hay fever, hives, eczema, pruritus, allergic rhinitis, allergic conjunctivitis, skin reaction after an insect bite; bronchial asthma (as part of combination therapy).

    Contraindications:Hypersensitivityprostate hyperplasia, angle-closure glaucoma, peptic ulcer and duodenal ulcerother inflammatory diseases of the digestive tract, pyloric stenosis, cardiac rhythm disturbances (having a vagolytic effect, it can improve conductivity in the atriapoint node and contribute to the development of supraventricular arrhythmias), epilepsy, pregnancy, breast-feeding.
    Carefully:

    Hepatic and / or renal failure (dose adjustment and interval between doses may be required).

    With caution, appoint patients who engage in potentially hazardous activities requiring increased attention and speed of psychomotor reactions.

    Pregnancy and lactation:

    The category of FDA recommendations is not defined.

    In pregnancy and lactation, the use is contraindicated.

    Dosing and Administration:Inside (after meal). Adults and children older than 10 years - 50-200 mg 1-2 times a day; maximum doses: single dose - 300 mg, daily - 600 mg; children from 5 to 10 years - 100-200 mg per day, from 2 to 5 years - 50-150 mg per day, up to 2 years - 50-100 mg per day.
    Side effects:

    From the nervous system and sensory organs: increased fatigue, dizziness, paresthesia; when using high doses - slowing the reaction rate, drowsiness, blurred vision, in children - paradoxical reactions: irritability, increased excitability, sleep disturbance, tremor.

    On the part of the digestive system: rarely dry mouth, nausea, heartburn, irritation of the gastric mucosa, pain in the epigastric region, dyspeptic phenomena, vomiting, constipation.

    From the urinary system: violation of urination.

    From the hematopoiesis: rarely - granulocytopenia, agranulocytosis.

    Overdose:

    Anticholinergic effects: clumsiness or instability, dryness in the oral cavity, nose or throat, flush of blood to the face, shortness of breath.

    CNS depression (drowsiness) or CNS stimulation (hallucinations, convulsions, restless sleep).

    Hypotension.

    Treatment is symptomatic.Do not use respiratory analeptics due to the threat of seizures.

    Interaction:

    Alcohol and drugs that depress the central nervous system (barbiturates, hypnotics, narcotic analgesics, sedatives, anxiolytics and antipsychotics) -increase CNS depression.

    Allergens used for carrying out skin tests - to cancel antihistamines before carrying out skin tests due to the possibility of false negative results.

    Anticholinergics or other drugs with anticholinergic activity - increased anticholinergic effects, the development of paralytic intestinal obstruction.

    Apomorphine - a decrease in the effects of apomorphine.

    MAO inhibitors, including furazolidone and procarbazine - prolongation and intensification of the inhibitory effect on the CNS and the anticholinergic effects of the mebhydroline.

    Drugs that have ototoxic effect - masking symptoms of ototoxic action, such as tinnitus or dizziness.

    Photosensitizing agents - enhanced photosensitization.

    Special instructions:

    During the treatment period, one should refrain from engaging in potentially hazardous activities,requiring increased attention and speed of psychomotor reactions (driving vehicles or working with mechanisms).
