    A.11.H.A.02   Pyridoxine (vitamin B6)

    Pharmacodynamics:

    Pyridoxine (vitamin B6), is involved in the metabolism; is necessary for the normal functioning of the central and peripheral nervous system. Entering the body, it is phosphorylated, converted to pyridoxal-5-phosphate and is part of the enzymes that carry out decarboxylation and reamination of amino acids. Participates in the exchange of tryptophan, methionine, cysteine, glutamic and other amino acids. It plays an important role in the exchange of histamine. Promotes the normalization of lipid metabolism. The drug increases diuresis and intensifies the action of diuretics. Isolated pyridoxine deficiency is very rare, mainly in children who are on special artificial nutrition (manifested by diarrhea, seizures, anemia, peripheral neuropathy may develop).

    Pharmacokinetics:

    It is absorbed quickly all over, the small intestine, more is absorbed in the jejunum.

    Metabolised in the liver with the formation of pharmacologically active metabolites (pyridoxal phosphate and pyridoxamine phosphate). Pyridoxal phosphate with plasma proteins binds to 90%. It penetrates well into all tissues; accumulates mainly in the liver, less in the muscles and central nervous system. Penetrates through the placenta, is secreted with breast milk. The half-life is 15-20 days. It is excreted by the kidneys, as well as during hemodialysis.

    Indications:

    Treatment of hypo- and avitaminosis B6.

    In complex therapy:

    • diseases of the nervous system (neuralgia, Meniere's syndrome);
    • in dermatology (dermatitis, including atopic and seborrheic, shingles, neurodermatites, psoriasis, exudative diathesis);
    • sideroblastic anemia;
    • acute and chronic hepatitis.

    Contraindications:

    Hypersensitivity to the components that make up the drug; Children's age (for this dosage).

    Carefully:

    Peptic ulcer of the stomach and duodenum (due to the possible increase in the acidity of the gastric juice), coronary heart disease, severe liver damage.

    Pregnancy and lactation:

    In this dosage is not recommended for use during pregnancy and breastfeeding.

    Dosing and Administration:

    Inside (after eating).

    For the prevention of B6-hypovitaminosis in adults - 5 mg / day.

    Inside the drug is taken after a meal. Therapeutic doses for oral administration are 20-30 mg for adults 1-2 times a day. The course of treatment is 1-2 months.

    When using isoniazid, ftivazid or other derivatives of hydrazide isonicotinic acid, it is advisable to prescribe pyridoxine 5-10 mg per day prophylactically (to prevent violations of the central nervous system).

    For treatment of sideroblastic anemia, 100 mg daily is administered.

    It is advisable to simultaneously take folic acid, cyanocobalamin, riboflavin.

    Side effects:

    Possible allergic reactions, increased acidity of gastric juice, pain in the epigastric region, the appearance of a feeling of compression in the extremities - a symptom of "stocking" and "gloves," a decrease in lactation.

    Interaction:

    Strengthens the action of diuretics; weakens the activity of levodopa.

    Isonicotin hydrazide, penicillamine, cycloserine and estrogen-containing oral contraceptives weaken the effect of pyridoxine.

    Combines with cardiac glycosides (pyridoxine promotes an increase in the synthesis of contractile proteins in the myocardium), with glutamic acid, and also potassium and magnesium asparaginate (asparcam).

    Pyridoxine prevents or reduces toxic manifestations, including liver damage, when using isoniazid and other anti-tuberculosis drugs.

    Special instructions:

    The need for vitamin B6 is satisfied by food: in part it is also synthesized by the intestinal microflora.

    The daily need for pyridoxine for adults is 2-2.5 mg. For women - 2 mg and additionally with pregnancy 0.3 mg, with breastfeeding - 0.5 mg.

    With severe liver damage pyridoxine in large doses can cause a deterioration in its function.

    The composition of the drug contains glucose, which should pay attention to patients with diabetes.

    When determining urobilinogen using Ehrlich's reagent, it can distort the results.
