    Pharmacodynamics:

    Hydrocortisone is a synthetic glucocorticosteroidpreparationvolume for local use.

    Has anti-inflammatory, anti-edematous, antipruritic effect.

    Pharmacokinetics:

    After application, it accumulates in the epidermis (mainly in the granular layer).

    Metabolized directly in the epidermis, and later in the liver.

    It is allocated, kidneys and with bile.
    Indications:Hydrocortisone ointment 1 % is used for inflammatory and allergic skin diseases of non-microbial etiology: eczema, dermatitis (allergic, contact); diffuse neurodermatitis, itching dermatosis, photodermatosis, anogenital itching, insect bites, erythroderma, psoriasis.
    Contraindications:

    The use of the drug is contraindicated in cases of hypersensitivity to hydrocortisone or other components of the drug, bacterial, viral and fungal skin diseases, skin tuberculosis, syphilitic skin lesions.

    Tumors of the skin, violation of the integrity of the skin (ulcers, wounds); rosacea, acne vulgaris, perioral dermatitis, post-vaccination period, children's age (up to 2 years, with an itch in the anus area - up to 12 years).
    Carefully:

    Diabetes mellitus, tuberculosis (systemic damage).

    With special care, the drug should be applied to the skin of the face due to the possibility of side effects (telangiectasia, atrophy, perioral dermatitis), even after short-term use. To prevent infectious skin lesions, hydrocortisone ointment is recommended to be prescribed in combination with antibacterial and antifungal agents.
    Pregnancy and lactation:
    The use of corticosteroids during pregnancy is possible if the expected effect of therapy for the mother exceeds the potential risk for theyes (adequate and strictly controlled safety studies applynot conducted). Women of childbearing age need to be warnedto wait for potential risk for the fetus (glucocorticosteroids undergo through the placenta). It is necessary to carefully monitor newborns, whose mothers received glucocorticosteroids during pregnancy (possibly development of adrenal insufficiency in the fetus and newborn). The drug should not be used often, in high doses; for a long timetime period.

    Breastfeeding women are recommended to stop either breastfeeding or the use of medicinal product (LS), especially in high doses (glucocorticosteroids penetrate into breast milk and can suppress growth, the production of endogenous glucocorticosteroids and cause unwantednesseffects on the newborn). When using the drug is not followedblows on the skin of the breast.

    Dosing and Administration:
    Outwardly.Ointment is applied a thin layer on the affected skin 2-3 times a day. The duration of treatment depends on the nature of the disease and the effectiveness of therapy, usually 6-14 days; with persistent - the course of the disease, it can be extended to 20 days. The dose of the drug used within 1 week, should not exceed 30-60 g. If itching in the anus area before applying the drug, wash the affected area with warm water with soap, then with water and dry the surface of the skin with a towel or napkin. On limited foci, occlusal dressings can be used to enhance the effect.

    When using the drug in children from 2 years and older, the total duration of treatment should be limited and the measures leading to increased resorption of steroid absorption (warming, fixing and occlusive dressings) should be excluded.

    In childhood, suppression of the function of the adrenal cortex can develop faster. In addition, there may be a decrease in hormone excretionon growth. When using the drug for a long time, it is necessaryto observe the body weight, height, plasma cortisol level.

    Side effects:

    There may be phenomena of individual intolerance in the form of hyperemia, swelling and itching in the places of ointment application. With prolonged use, it is also possible to develop secondary infectious skin lesions, atrophic changes in it, hypertrichosis. To prevent infectious skin lesions, the drug is recommended to be prescribed in combination with antibacterial and antifungal agents.

    With prolonged use of ointment or the use of an occlusive dressing, especially on large areas of damage, it is possible to develop hypercorticism, as a manifestation of the resorptive action of hydrocortisone.

    Overdose:

    Acute overdose is unlikely, however, if excessive or longChronic overdose is possible, accompanied by signs of hypercorticism: hyperglycemia; glucosuria, reversible oppression of adrenal cortex function, manifestation of Cushing's syndrome.

    Treatment. The corresponding symptomatic treatment is shown! In the case of chronic toxic effects, a gradual withdrawal of the drug is recommended.

    Interaction:

    With prolonged use of the drug due to possible resorption in the systemic circulation, hydrocortisone can reduce the effectiveness of insulin, oral hypoglycemic, antihypertensive drugs, anticoagulants, concentration of salicylates and praziquantel in blood plasma.

    The risk of side effects of hydrocortisone is increased: androgens, estrogens, oral contraceptives, anabolic steroids (hirsutism, acne), antipsychotic drugs, carbutamide, azathioprine (cataract), holinoblokatory, antihistamine drugs, tricyclic antidepressants, nitrates (glaucoma); diuretics (hypokalemia), non-steroidal anti-inflammatory drugs (increased risk of ulceration in the gastrointestinal tract), paracetamol (hepatotoxicity). When used with cardiac glycosides, it is possible to develop digitalis intoxication. The simultaneous use of lead ions and silver ions with preparations leads to mutual inactivation with amphotericin B, possibly dilatation of the myocardium and heart failure. Barbiturates, antiepileptic and antihistamines reduce the effectiveness of hydrocortisone.

    Special instructions:Avoid contact with ointment in the eyes.Children under 12 years of age, the drug is prescribed only under strict medical supervision. If after 7 days of application there is no improvement or if there is a worsening of the condition, and also if the symptoms recur after a few days after cancellation, the drug should be discontinued and consulted by a doctor. Do not use the drug without consulting a doctor in cases where itching in the genital area of ​​women is accompanied by vaginal discharge. With long-term treatment and / or application to large surfaces, it is recommended to prescribe a diet with sodium restriction and high potassium content; to introduce into the body a sufficient amount of protein. It is necessary to control blood pressure, the concentration of glucose and cortisol in the blood, blood clotting, diuresis, body weight of the patient.
