    Pharmacodynamics:
    Vitamin B1 (thiamine) refers to water-soluble vitamins. In the human body, as a result of phosphorylation processes, it turns into cocarboxylase, which is a coenzyme of many enzymatic reactions.It plays an important role in the carbohydrate, protein and fat metabolism, in the processes of conduction of nervous excitation in the synapses.

    Pharmacokinetics:
    . Absorption is high, throughout the small intestine. In the blood, the concentration is relatively low, while the plasma circulates predominantly free thiamine, in erythrocytes and leukocytes - its phosphoric esters. Phosphorylation occurs in the liver. The most active phosphoric ester is thiamine diphosphate, which has coenzymic activity. It accumulates mainly in the liver, heart, brain, kidneys, spleen. 1/2 of the total is contained in striated muscle and myocardium and about 40% in internal organs. It is excreted through the intestines and kidneys.

    Indications:
    Hypovitaminosis and vitamin B1.
    In complex therapy:
    neuritis, sciatica; neuralgia; peripheral paresis and paralysis;
    - ulcer disease of the stomach and duodenum; decrease in secretory and motor function of the stomach, anorexia, intestinal atony; atonic constipation;
    - Myocardial dystrophy; violation of the coronary circulation;
    -diabetes;
    dermatoses (eczema, neurodermatitis, psoriasis,lichen planus) with neurotrophic changes and metabolic disorders.

    Contraindications:
    Increased individual sensitivity.

    Dosing and Administration:
    Thiamine solution 5% is administered intramuscularly (deep into the muscle).
    Begin the administration of the drug is recommended with small doses (no more than 0.5 ml of a 5% solution) and only with good tolerance go to higher doses.
    Adults appoint 50 mg thiamine chloride (1 ml of 5% solution) once a day, daily. The course of treatment - 10-30 injections.

    Side effects:
    Allergic reactions are possible (skin itching, urticaria, Quincke edema, anaphylactic shock), increased sweating, tachycardia, sometimes soreness at the injection site (due to low pH of the solution).

    Interaction:
    It is not recommended simultaneous parenteral administration of thiamine and pyridoxine (vitamin B6) or cyanocobalamin (vitamin B12). B12 increases the allergic effect of thiamine. In 6, it makes it difficult to convert thiamine into a biologically active form. Do not mix vitamin B1 in one syringe and benzylpenicillin or streptomycin (destruction of antibiotics), vitamin B1 and nicotinic acid (destruction of vitamin B1).
    Thiamine reduces the pharmacological activity of suxamethonium iodide, phentolamine, propranolol, sympatholytics (reserpine), hypnotics (hypnotics and hypotensive effects are reduced).
    Thiamine solution should not be mixed with solutions containing sulfites, t. in them it completely disintegrates.
    Thiamine is unstable in alkaline and neutral solutions; the appointment with carbonates, citrates, barbiturates, copper preparations is not recommended.

    Special instructions:
    When determining theophylline in the blood serum by the spectrophotometric method, urobilinogen with the help of Ehrlich reagent may distort results (when taking high doses). More often the anaphylactic reaction develops after intravenous administration of large doses.
    Parenteral administration is recommended only if oral administration (nausea, vomiting, malabsorption syndrome, preoperative and / or postoperative conditions) is not possible.
    With encephalopathy Wernicke, the appointment of dextrose should precede the administration of thiamine.
    Children should not use a 5% solution,because low dosages are used for this category of patients (0.0125 g thiamine chloride, ie 0.5 ml 2.5% solution), which is difficult to obtain from a 5% solution.
