  • Dosage form: & nbspsolution for intramuscular injection
    Composition:
    1 ml of the solution contains:
    active substance: thiamine hydrochloride - 50 mg;
    auxiliary substances: sodium dimercaptopropanesulfonate monohydrate (unitiol) - 2 mg, disodium edetate - 0.2 mg, water for injection.

    Description:
    Colorless or slightly colored transparent liquid with a weak characteristic odor.

    Pharmacotherapeutic group:vitamin
    ATX: & nbsp

    A.11.D.A.01   Thiamine

    Pharmacodynamics:
    Vitamin B1 (thiamine) refers to water-soluble vitamins.In the body, as a result of phosphorylation processes, it turns into cocarboxylase, which is a coenzyme of many enzyme reactions.
    It plays an important role in the carbohydrate, protein and fat metabolism, as well as in the processes of conduction of nervous excitation in the synapses.

    Pharmacokinetics:
    In the blood, the concentration is relatively low, while the plasma circulates predominantly free thiamine, in erythrocytes and leukocytes - its phosphoric esters. Phosphorylation occurs in the liver. The most active phosphoric ester is thiamine diphosphate, which has coenzymic activity. It accumulates mainly in the liver, heart, brain, kidneys, spleen. 1/2 of the total amount is contained in striated muscles and myocardium and about 40% in internal organs. It is excreted through the intestines and kidneys.

    Indications:
    Hypovitaminosis and vitamin B1.
    As part of complex therapy:
    - neuritis, radiculitis, neuralgia; peripheral paresis and paralysis;
    - peptic ulcer of stomach and duodenum; anorexia, intestinal atony, atonic constipation;
    - myocardial dystrophy; violation of the coronary circulation;
    - dermatoses (eczema, neurodermatitis, psoriasis, lichen planus) with neurotrophic changes and metabolic disorders.

    Contraindications:
    Increased individual sensitivity, pregnancy, lactation, children's age.

    Carefully:Encephalopathy Wernicke, a woman in the premenopausal and climacteric period.
    Dosing and Administration:
    Thiamine solution is administered intramuscularly (deep into the muscle).
    It is recommended to start the drug administration from small doses (no more than 0,5 ml of solution) and only with good tolerance go to higher doses.
    Adults with 20-50 mg thiamine chloride (0.5-1 ml) once a day. The course of treatment is 10-30 injections.

    Side effects:
    Allergic reactions are possible (hives, skin itching, Quincke's edema, anaphylactic shock), increased sweating, tachycardia.
    Sometimes soreness (due to low pH of the solution) at the site of administration.
    Interaction:
    It is not recommended simultaneous parenteral administration of thiamine and pyridoxine (vitamin Bb) or cyanocobalamin (vitamin Bi2). B12 increases the allergic effect of thiamine. Bb makes it difficult to convert thiamine into a biologically active form.Do not mix vitamin Bi in one syringe and benzylpenicillin or streptomycin (destruction of antibiotics), vitamin Bi and nicotinic acid (destruction of vitamin Bi). Thiamine reduces the pharmacological activity of suxamethonium iodide, phentolamine, propranolol, sympatholytics (reserpine), hypnotics (hypnotics and hypotensive effects decrease).
    Thiamine solution should not be mixed with solutions containing sulfites, t. in them it completely disintegrates.
    Thiamine is unstable in alkaline and neutral solutions; the appointment with carbonates, citrates, barbiturates, copper preparations is not recommended.

    Special instructions:
    When determining theophylline in serum by the spectrophotometric method, urobylinogen with the help of Ehrlich reagent can distort the results (when taking high doses). More often the anaphylactic reaction develops after intravenous administration of large doses.

    Parenteral administration is recommended only if oral administration (nausea, vomiting, malabsorption syndrome, preoperative and / or postoperative conditions) is not possible.
    With encephalopathy Wernicke, the appointment of dextrose should precede the administration of thiamine.

    Form release / dosage:
    Solution for intramuscular injection 50 mg / ml.
    Packaging:
