Oxygen medical gaseous - instructions for use, dose, side effects, contraindications

Antihypoxic agent, increases oxygenation of tissues, improves hemodynamics and protects the brain from hypoxia. Has a metabolic and anthelmintic effect. In conditions of normobaric hypoxia, it increases the resistance of the organism to endogenous and exogenous pathological factors.

Indications:

Hypoxia: with respiratory diseases (including pneumonia, bronchial asthma, pulmonary edema), cardiovascular system (including chronic heart failure, cardiopulmonary insufficiency, collapse, arrhythmia).

Weakened breathing in the postoperative period.

Poisoning with carbon monoxide, hydrocyanic acid, asphyxiating substances (including chlorine, phosgene).

Ischemia of the lower extremities, trauma of the limbs.

Vascular diseases of the brain.

Hyperbaric oxygenation (heart and lung operations, reconstructive operations on the digestive tract, intoxication, anemia, peptic ulcer and duodenal ulcer, chronic hepatitis).

Hypoxicotherapy (coronary heart disease, arterial hypertension, gastrointestinal diseases, asthenia, depression, fatigue, condition after radiation therapy).

Contraindications:

Hyperbaric oxygenation (HBO) is contraindicated in:

- chronic and acute inflammatory processes in the nasopharynx, bronchi, lungs, ears (rhinitis, sinusitis, bronchitis, runny nose, tonsillitis, otitis, poor tolerance of flights in the airplane);

- presence of cysts and cavities in the lungs, bronchopleural fistula;

- epileptic seizures or their equivalents (after head injuries, vascular, infectious brain damage);

- persistent increase in blood pressure above 160/90 mm Hg. and severe forms of hypertension;

- glaucoma, hemorrhages (fresh) on the fundus and proliferative changes in the vessels of the fundus (for diabetics it is necessary to have an ophthalmologist and a permit for HBO);

- active bleeding (HBO is possible only after stopping it);

- fear of closed space (claustrophobia);

- hypersensitivity to oxygen;

- neoplasms.

Carefully:

HBO is administered with caution in bronchial asthma, congenital spherocytosis, high fever, optic neuritis,infection of the upper respiratory tract, pregnancy, simultaneous administration of cisplatin, doxorubicin, disulfiram, mafenide (oxygen increases the toxicity of the drug data), emphysema, barotrauma of the ears.

Pregnancy and lactation:

Oxygen therapy can be used if the potential benefit to the mother exceeds the possible risk to the fetus or the baby.

Dosing and Administration:

Inhalation, in a concentration of 40-60%, in a mixture with air, in an amount of 4-5 l / min.

When weakening of respiration in the postoperative period, poisoning, intoxications use 100% oxygen or a mixture of carbonic acid. Inhalation is carried out through masks or tubes, attached by means of special devices to a cylinder with oxygen, or using rubber cushions.

Hyperbaric oxygenation: The procedure is carried out in special pressure chambers, for therapeutic purposes apparatuses (single-chamber chambers) are used that create an oxygen pressure of 1.2-1.6-2 atm. Conduct 1 session per day (40-60 min), treatment course 8-10 sessions.

With gastric ulcer and duodenal ulcer, chronic hepatitis is injected under a pressure of 0.7-1 excess atm. (sessions for 45 minutes); course of treatment - 15 sessions.

Hypoxicotherapy: special training that leads to oxygen deficiency (normobaric hypoxia) by using gas mixtures with a reduced oxygen content to create an artificial hypoxia (at normal atmospheric pressure).

Side effects:

WHO classification of unwanted adverse reactions according to the frequency of development: very frequent - 1/10 appointments (> 10%), frequent - 1/100 appointments (> 1% and <10%), infrequent - 1/1000 appointments (> 0.1% and <1%), rare - 1/10000 appointments (> 0.01% and <0.1%), very rare - 1/10000 prescriptions (<0.01%).

With hyperbaric oxygenation: hypoglycemia, headache, numbness of fingers, transient visual impairment, ear barotrauma (ear pain, damage and rupture of the eardrum) and paranasal sinuses, fatigue, acceleration of cataract ripening; rarely - acute oxygen intoxication (convulsions, pulmonary edema).

With oxygen therapy: Possible myocardial ischemia in patients who underwent myocardial infarction.

Overdose:

Symptoms: dry mouth, dry cough, cramps, burning behind the sternum, impaired heat regulation.

Treatment: symptomatic.

Interaction:

Oxygen increases the toxicity of cisplatin, doxorubicin, disulfiram, mafenide.

Special instructions:

With prolonged and unsystematic application of high concentrations of oxygen (over 60-80%) with a large flux (over 6-8 l / min) there is a danger of hyperoxygenation and as a consequence - pulmonary edema.

With prolonged action of oxygen, decompensatory reactions develop, manifested by functional and structural disorders in various systems and organs.

Signs of oxygen intoxication appear after a certain period. Its duration may depend on individual sensitivity to oxygen, temperature and humidity of the environment, concentration in the inhaled gas mixture, emotional and physical loads, the state of the central nervous system, etc.

Packaging:

6,2 m³ in a steel cylinder of blue color with a capacity of 40 liters, designed for working pressure of 14.7 MPa at a temperature of 20 ° C, equipped with a first-opening control device with a black inscription "OXYGEN MEDICAL" on the circumference.

Each cylinder is supplied with instructions for the medical use of the drug, which is attached to the top of the cylinder on the valve with the help of a wire and a seal.

Storage conditions:
