    D.08.A.C.02   Chlorhexidine

G.01.A.X   Other antiseptics and antimicrobials for the treatment of gynecological diseases

Pharmacodynamics:Antiseptic and disinfectant. Depending on the concentration used, it shows both bacteriostatic and bactericidal action Chlorhexidine Bigluconate is a cationic biguanide. Chlorhexidine blocks amino groups of cellular proteins. It penetrates the intracellular membranes of bacterial cells, precipitates on the cytoplasm and is implanted in the membrane function, preventing the consumption of oxygen, which leads to a decrease in the level of ATP and cell death. Destroys DNA and disrupts DNA synthesis in microorganisms. Provides long-lasting persistent antimicrobial activity, which prevents reproduction of microorganisms for at least 6 hours after application of the drug. In the presence of blood and other organic substances, the antimicrobial activity of chlorhexidine digluconate does not decrease. Effective against causative agents of sexually transmitted infections - gardnerelosis, genital herpes; Gram-positive and Gram-negative bacteria - Treponema spp., Neisseria gonorrhoeae, Trichomonas spp., Chlamidia spp., Ureaplasma spp. He acts on acid-fast forms of bacteria, microbial spores, fungi. Stable, after treatment of the skin (hands, operating field) is stored on it in a certain amount, sufficient for the manifestation of bactericidal effect. Saves activity (although somewhat reduced) in the presence of blood, pus, various secrets and organic substances.
Very rarely causes allergic reactions, skin and tissue irritation, does not have a damaging effect on objects made of glass, plastics and metals.
Pharmacokinetics:With local and external application is almost not absorbed.
    Indications:

As a therapeutic and prophylactic for various infections.

Applied in the treatment of purulent-septic processes, for disinfection of microtraumas (wounds, scratches, burns), as well as for the prevention of sexually transmitted diseases (syphilis, gonorrhea, trichomonas).

Contraindications:Individual hypersensitivity to chlorhexidine. Propensity to allergic reactions and allergic diseases, dermatitis.
    Carefully:Children's age, pregnancy, lactation.
Pregnancy and lactation:Application during pregnancy and breastfeeding is possible if the proposed benefit to the mother exceeds the potential risk to the fetus or child.
Dosing and Administration:Outer, local. At microtrauma, wounds, scratches, burns, 5-10 ml of the solution is applied to the affected skin surface with an exposure of 1-3 minutes 2-3 times per day (on a tampon or by irrigation). Before using the drug to prevent venereal diseases, you should urinate, wash your hands and genitals. After this, remove the cap, press on the walls of the vial and spray the solution of the pubic skin, internal surfaces of the thighs, genitals. In the external opening of the urethra should be introduced nozzle nozzle, tightly press the walls of the urethra to it and squeeze out 1.5-3 ml of solution (for men) or 1-1,5 ml of solution (for women). Then, without parting fingers, pull the nozzle out of the urethra, and the solution is held for 2-3 minutes. The solution that remained, the women sprayed the vagina. After the procedure, it is recommended not to urinate for 2 hours.Antiseptic treatment of the skin and mucous membranes with chlorhexidine is effective if it is performed no later than 2 hours after sexual intercourse.
    Side effects:

In places of application of the preparation, redness, sensation of itching or burning, skin rash, dry skin, dermatitis, photosensitivity may appear. Allergic reactions.

    Overdose:

Cases of overdose with external application of chlorhexidine are not known. In case of accidental ingestion, it is practically not adsorbed, it is necessary to rinse the stomach using milk, gelatin or a raw egg. If necessary, symptomatic therapy is performed.

Interaction:To avoid the danger of developing dermatitis, simultaneous use of chlorhexidine and iodine preparations is undesirable. Chlorhexidine Bigluconate is a cationic substance and does not combine with soap and other anionic substances (colloids, gum arabic, carboxymethylcellulose). Compatible with drugs that contain a cationic group (benzalkonium chloride, centrimony bromide). Ethanol enhances the effect of the drug. At a concentration of 0.05% chlorhexidine It is incompatible with borates, carbonates, chlorides, citrates, phosphates, sulfates, creating sparingly soluble deposits after 24 hours. It is used in a neutral environment. At a pH of more than 8, a precipitate forms.
    Special instructions:

The drug is intended for external use only. Avoid contact with eyes, as it can cause irritation. In case of accidental ingestion of the drug in the eye, they should be rinsed with running water.

In patients with open craniocerebral trauma, spinal cord injuries, perforation of the tympanic membrane, one should avoid getting onto the surface of the brain, the cerebral membranes and into the cavity of the inner ear.
